BackgroundAcute upper respiratory infections (AURI) are the leading causes of antibiotic prescribing in primary care although antibiotics are often not indicated. AimTo gain an understanding of the knowledge, attitudes, and practices (KAP) of GPs in Singapore and the associated latent factors to guide the implementation of an effective programme to reduce antibiotic use in primary care. Design and settingAn anonymous survey on the KAP of antibiotic use in AURI of GPs in Singapore. MethodKAP survey questionnaires were posted to all GPs from a database. To ascertain the latent factors affecting prescribing patterns, exploratory factor analysis was performed. ResultsAmong 427 responses, 351 (82.2%) were from GPs working in private practice. It was found that 58.4% of GPs in the private versus 72.4% of those in the public sector recognised that >80% of AURIs were caused by viruses (P = 0.02). The majority of GPs (353/427; 82.7%) felt that antibiotics were overprescribed in primary care. Significant factors associated with low antibiotic prescribing were good medical knowledge and clinical competency (adjusted odds ratio [aOR] 3.2, 95% confidence interval [CI] = 2.4 to 4.3), good clinical practice (aOR 2.7 [95% CI = 2.0 to 3.6]), availability of diagnostic tests (aOR 1.4 [95% CI = 1.1 to 1.8]), and desire to improve clinical practice (aOR 1.5 [95% CI = 1.2 to 1.9]). The conservative practice of giving antibiotics 'to be on the safe side' is significantly less likely to be associated with low antibiotic prescribing (aOR 0.7 [95% CI = 0.5 to 0.9]). ConclusionThis is the first KAP survey on antibiotic prescribing for AURI among GPs in Singapore. With the latent factors identified, future interventions should be directed at addressing these factors to reduce inappropriate antibiotic prescribing.

Objective: Anti-microbial resistance has become a global problem especially in developing countries. The aim of the study was to evaluate the effect of socio-demographic predictors amongst final year pharmacy undergraduates in Sri Lanka on their knowledge, perception and attitudes regarding antibiotic use. Methods:A questionnaire-based study involved final year pharmacy students from four public universities in Sri Lanka. Data on knowledge, attitudes and perception about the appropriate use of antibiotics were analysed. Ethics approval was granted by the university of Ruhuna Sri Lanka. Results:There was good general knowledge of antibiotics (mean=15.57), however, 46% said metronidazole is not an antibiotic, 82% said that the use of antibiotics speed-up the recovery from the common cold. Perception (p=0.033) and attitudes (p=0.028) of respondents from urban area were significantly higher than that of those from rural area. Conclusion:Clinical education for pharmacy undergraduates should have an in-depth focus on the rational of antibiotics use in Sri Lanka.

Резюме. Описано особливості етіології та клінічного перебігу гострих респіраторних інфекційних захво-рювань верхніх дихальних шляхів у дітей порівняно з дорослими, оцінено особливості їх клінічних проявів, диференціальної діагностики, тактики ведення та ускладнень у дитячому віці.Ключові слова: гострі інфекційні захворювання верхніх дихальних шляхів, діти, етіологія, клінічні прояви.Респіраторні інфекції щорічно зумовлюють більше половини всіх випадків гострих захворювань серед ді-тей [20]. Найчастіше серед цих захворювань -звичай-на застуда -гостре респіраторне захворювання (ГРЗ) верхніх дихальних шляхів, переважно вірусної етіо-логії. Будь-які респіраторні мікроорганізми можуть викликати захворювання верхніх дихальних шляхів, що майже не різняться за клінічною симптоматикою (закладеність носа, риніт, відсутність вираженого фа-рингіту та невисока або відсутня лихоманка) [17]. Діти особливо сприйнятливі до цих захворювань внаслідок обмеженості набутого імунітету проти багатьох збудни-ків, недостатніх гігієнічних навичок та частого тісного контакту з оточенням -хворими або носіями інфек-ційних збудників [22].Риновіруси, коронавіруси, респіраторно-синциті-альний вірус (РСВ), віруси грипу, парагрипу, людський метапневмовірус, аденовіруси, ентеровіруси -загалом більше ніж 200 вірусів можуть викликати подібні симп-томи. Зрідка (менше ніж у 10 % випадків) ГРЗ спри-чиняють бактерії, мікоплазми, грибки, найпростіші, може відбутися коінфікування. У дітей ГРЗ трапля-ються частіше у зв'язку з нетривалістю імунітету піс-ля перенесеного захворювання та великою кількістю різних серотипів вірусів [19]. Незважаючи на невелику кількість різних серотипів, повторне інфікування час-то спричиняється зараженням РСВ, вірусами парагри-пу, людським метапневмовірусом та коронавірусами, проти яких імунітет нетривкий [16]. Велика кількість серотипів вірусів грипу, рино-, адено-та ентеровірусів також є причиною реінфікувань, хоча серотипспеци-фічний імунітет проти цих збудників здебільшого є тривким (табл. 1). Взаємодія коронавірусів та організ-му не досліджена глибоко, але багато з них викликають принаймні нетривалий імунний захист. Респіраторні віруси виробляють різні механізми уникнення імунно-го захисту організму. Віруси грипу змінюють поверхне-ві антигени та уподібнюються різним серотипам.Загалом риновіруси відповідальні принаймні за 50 % усіх застудних захворювань у дітей і дорослих, на-самперед коронавіруси, РСВ, віруси грипу, парагрипу, людський метапневмовірус, аденовіруси, ЕСНО-віру-си, віруси Коксакі. На відміну від дорослих в етіологіч-ному спектрі ГРЗ у дитячому віці істотно частіше спо-стерігаються бокавіруси та Moraxella catarrhalis, а також коінфікування різними вірусами [8].Деякі віруси зумовлюють характерні клінічні ознаки. Зокрема, РСВ переважно викликає бронхіо-літ у дітей до 2 років, вірус грипу -респіраторне захворювання з високою лихоманкою, аденовіру-си -фарингокон'юнктивальну лихоманку, віруси па-рагрипу -круп у дітей раннього віку, ентеровіруси -різні захворювання, зокрема й асептичний менінгіт та герпангіну [13,20...
