Relevance. Interstitial cystitis/painful bladder syndrome (IC/PBS) is a debilitating condition of pain and discomfort in the bladder. Due to the limited number of published studies in this area, there is a need for further research. Objective: to evaluate the level of nerve growth factor and its relationship with various types of leukocytes and mast cells of the bladder tissue in animals with experimental models of interstitial cystitis/painful bladder syndrome. Material and methods. IC/PBS modeling was performed on 38 female rabbits. The IC/PBS modeling was created by introducing HCl into the bladder (group 1), urine into the wall of the bladder (group 2), and physiological saline (group 3). Nerve growth factor (NGF) was determined by the ELISA method, and white blood cells along with mast cells in tissues were determined by histological processing. Results. A statistically significant high level of NGF in blood and urine was observed on the 1st day after IC/PBS initiation in groups 13 and in group 2 respectively. Two weeks after the initiation of IC/PBS in animals of group 1 a decrease in the level of FRN in the blood by 29.3% and its increase in urine by 14.3% was observed; in group 2 an increase by 65.5% (p 0.01) in blood and by 52.7% (p 0.01) in urine was observed and in group 3 a decrease by 30.8% (p 0.05) in blood and by 30.5% (p 0.05) in urine was observed. The greatest number of leukocytes was determined in the tissue of the bladder in animals of the 1st group. Mast cell infiltration was observed in groups 1 and 2. Correlation between the NGF in blood and urine and between NGF and the number of leukocytes and mast cells was revealed. Findings. In IC/PBS, the level of NGF in the blood and urine is increased. Indicators of nerve growth factor in blood and urine are correlated with multidirectional connections. High mast cell infiltration occurs when damage to the integrity of the bladder by urinary toxicity. Nerve growth factor correlates with leukocyte and mast cell infiltration in the bladder tissue. Conclusion. During IC/PBS, the level of NGF in the blood and urine is increased. Levels of nerve growth factor in blood and urine are correlated with multidirectional connections. High mast cell infiltration occurs as a response to the damage of the bladder wall integrity caused by urinary toxicity. The level of nerve growth factor correlates with the bladder tissue intensity of infiltration with leukocytes and mast cells.

Pain associated a bony pathology puts a significant burden on individuals, society, and the health-care systems worldwide. Pathology that involves the bone marrow activates sensory nerve terminal endings of peripheral bone marrow nociceptors, and is the likely trigger for pain. This review presents our current understanding of how bone marrow nociceptors are influenced by noxious stimuli presented in pathology associated with bone marrow. A number of ion channels and receptors are emerging as important modulators of the activity of peripheral bone marrow nociceptors. Nerve growth factor (NGF) sequestration has been trialed for the management of inflammatory bone pain (osteoarthritis), and there is significant evidence for interaction of NGF with bone marrow nociceptors. Activation of transient receptor potential cation channel subfamily V member 1 sensitizes bone marrow nociceptors and could contribute to increased sensitivity of patients to noxious stimuli in various bony pathologies. Acid-sensing ion channels sense changes to tissue pH in the bone marrow microenvironment and could be targeted to treat pathology that involves acidosis of the bone marrow. Piezo2 is a mechanically gated ion channel that has recently been reported to be expressed by most myelinated bone marrow nociceptors and might be a target for treatments directed against mechanically induced bone pain. These ion channels and receptors could be useful targets for the development of peripherally acting drugs to treat pain of bony origin.

Depletion of nerve growth factor in chemotherapy-induced peripheral neuropathy associated with hematologic malignancies

ObjectiveTo investigate whether the depletion of nerve growth factor (NGF) is associated with the development of chemotherapy-induced peripheral neuropathy (CIPN) in patients with hematologic malignancy.MethodsWe prospectively enrolled hematologic cancer patients who had a plan to receive bortezomib, thalidomide, or vincristine. Baseline NGF levels were measured within one week before the start date of chemotherapy. Follow-up NGF levels were measured after four months from the start date of chemotherapy or the date when CIPN was initially diagnosed.ResultsBaseline and follow-up NGF pairs were measured in 45 patients (male/female = 27/18, median age = 63 years old). CIPN has developed in 28 patients. In the CIPN group, the level of NGF was significantly decreased after chemotherapy compared to the baseline (△NGF = −3.52 ±5.72; p-value = 0.003), while the NGF level of the no-CIPN group was not changed after chemotherapy. The differences in △NGF levels between the CIPN and no-CIPN group were more profound when analyzed in the subgroup of newly diagnosed multiple myeloma patients (△NGF = −4.14 ± 4.87 pg/ml for the CIPN group and +2.52 ± 8.39 pg/ml for the no-CIPN group; p-value = 0.043).ConclusionsThis study shows that the depletion of NGF occurs during the development of CIPN, suggesting pathogenesis based on the role of NGF and therapeutic implications.
