ZusammenfassungBei der idiopathischen ventralen Rückenmarksherniation handelt es sich um einen sehr seltenen Defekt der thorakalen ventralen Dura, am häufigsten zwischen BWK 2 und BWK 8, mit konsekutivem Prolaps des Myelons in diesen Duradefekt hinein. Klinisch führt dies oft nach jahrelanger Anamnese zu einer progredienten, mehr oder weniger stark ausgeprägten Myelopathie, z. B. einem Brown-Séquard Syndrom oder einer Paraparese. Die Ursache des Duradefekts und der Mechanismus der Herniation sind nicht geklärt. Diskutiert werden Bandscheibenpathologien wie verkalkte Mikrosporne, Trauma, Entzündung, eine Bindegewebsschwäche und angeborene Dura-Duplikaturen. Die wichtigsten Differentialdiagnosen sind arachnoidale Zysten und arachnoidale Netze (arachnoidal web), die das Myelon bei jedoch intakter Dura nach ventral verlagern, Tumore sowie ischämische und traumatische Myelomalazien. Trotz exzellenter Bildgebung gelingt manchmal die sichere Diagnose einer Myelonhernie samt Duradefekt letztendlich erst intraoperativ. Die operative Versorgung ist, einschließlich auch der nur leicht symptomatischen Patienten, die Therapie der Wahl und stellt eine neurochirurgische Herausforderung dar. Ziel ist das Lösen des Rückenmarks mit, je nach Größe und Lage, entweder Verschluss, Augmentation oder Erweiterung des Duradefekts. Das Risiko für postoperative Defizite beträgt 5–12%. Von einem Behandlungserfolg wird meist bei einem Stopp der klinischen Progression ausgegangen, wobei bis zu 3/4 der Patienten eine Chance auf postoperative Besserung haben. Die operative Behandlung sollte mit intraoperativem Neuromonitoring (SSEP, MEP, D-Welle) in Zentren mit entsprechender Erfahrung erfolgen. Patienten, bei denen noch keine Operationsindikation gestellt wurde, sollten engmaschig angebunden werden, da der natürliche Krankheitsverlauf der idiopathischen ventralen Rückenmarksherniation meist progredient ist.

Background: Spinal dysraphism with a hamartomatous growth (appendix) of the spinal cord is better known as herniated spinal cord. There are many arguments in favour of considering it a developmental defect. From this point of view, it is a type of neural tube disorder. Neural tube disorders can be caused by multiple factors, including a genetic factor. A common genetic defect in patients with a spinal dysraphism with a hamartomatous growth of the spinal cord is sought for. Case presentation: In two patients with a symptomatic lesion and referred to an academic hospital a genetic analysis was performed after informed consent. Whole-exome analysis was performed. : Whole-exome analysis did not result in identification of a clinically relevant genetic variant.Conclusions: This the first study to investigate the genetic contribution to spinal dysraphism with a hamartomatous growth (appendix) of the spinal cord. We could not establish a genetic cause for this entity. This conclusion cannot be definitive due to the small sample size. However, the incidental occurrence, the lack of reports of inheritance of this disorder and the absence of contribution to syndromal disorders favours a defect of normal development of the spinal cord.

Key clinical messageSpinal cord herniation is an uncommon diagnosis. There should be a high index of suspicion to diagnose spinal cord herniation when a patient presents with incomplete neurological deficits. Surgical repair of the hernia can have postoperative complications with new neurological deficits and they should be considered during the treatment.AbstractA 37‐year‐old male presented with insidious onset upper back pain and altered sensations of pain and temperature over the right half of the body below the nipple for 2 months. MRI of the thoracic spine showed an anterolateral defect (left) at the level of T2‐T3 vertebra. The defect was covered by a dural graft and the wound was closed with a drain On the 3rd postoperative day, neurological weakness progressed to paraplegia. Patient was treated by exploration and decompression of the hematoma. The deficits were completely recovered at one‐month follow‐up. Patients with spinal cord herniation and neurologic deficits when treated timely have good outcomes.
