Purpose: To evaluate macular, peripapillary choroidal thickness and retinal nerve fiber layer (RNFL) thickness in different trimesters and compare the measurements with those of healthy non-pregnant women. Methods: A prospective comparative study included 45 healthy pregnant women in first trimester, 90 women in second, 45 women in third trimesters and 45 healthy no-pregnant women as the control group. Macular choroidal thickness was measured at the subfoveal, 1mm temporal and 1mm nasal to the fovea using enhanced depth imaging optical coherence tomography (EDI-OCT). Peripapillary choroidal thickness and RNFL thickness parameters were automatically calculated by the Spectralis OCT. These parameters were analyzed among groups. Results: The subfoveal, temporal and nasal choroidal thickness were all significantly thicker in second trimesters, comparing with those parameters in first, third trimesters and the control group (all P<0.05). The peripapillary choroidal thickness was significantly increased at temporal, nasal, nasal inferior, temporal inferior and global positions during pregnancy among the 4 groups (all P<0.05). The RNFL thickness was also significantly increased in pregnant women at nasal superior and nasal inferior quadrants.Conclusions: Our results suggested that macular choroidal thickening appeared in second trimester. And peripapillary choroidal thickness and RNFL thickness also became thickening in pregnant women comparing with no-pregnant women as a whole.

Introduction: preeclampsia is a hypertensive disorder characterized by hypertension after 20 weeks of gestation, in most cases associated with proteinuria, and may be related to target organ dysfunction. Objective: To perform a literature review regarding complications related to either neuroaxial techniques or general anesthesia in patients with PE. Methods: A literature search was carried out by consulting multiple databases in both Spanish and English, with different combinations of MeSH terms. An analysis and summary of the information reviewed was performed after careful evaluation of the literature. Results: Both neuraxial techniques and general anesthesia in patients with PE could lead to a series of complications which will vary in severity and frequency, with estimates of adverse events of any type of approximately 10% described in the literature. Among the complications that occur with the use of neuraxial techniques are: hypotension, urinary retention and post-puncture headache among others. On the other hand, some of the potential complications with the use of general anesthesia are the development of intracerebral hemorrhage due to increased blood pressure during laryngoscopy, development of pulmonary edema or difficulty with the airway management. The development of hemodynamic instability, drug allergy, nausea and vomiting, cardiac arrest, and mortality are potential complications of both anesthetic techniques. Conclusions: Neuraxial techniques are considered the method of choice even in patients with preeclampsia, as long as there is no contraindication for its use, considering the risk-benefit balance between the potential complications of both anesthetic techniques.

Introduction: Pregnancy imposes physiological stress and significantly changes human body. This change may have potential effects on visual system. Little is known about pregnancy effects on intraocular pressure (IOP) and glaucoma. Objectives: The present study tended to evaluate and compare the effect of normal vaginal delivery (NVD) versus cesarean section (CS) on IOP and retinal nerve fiber layer thickness (RNFLT) in healthy women. Patients and Methods: Sixty pregnant women participated in this prospective cross-sectional study. The participants were assigned to two groups; the first group consisted of 30 candidates for NVD and the second group of 30 candidates for CS. Then, IOP in three states before delivery, during delivery, and six weeks after delivery then RNFLT were measured and compared by optical coherence tomography (OCT) in two states before delivery and six weeks after delivery. Results: Based on the results, there was no significant difference in the RNFLT in left and right eyes, before and after delivery in the NVD and CS groups (P=0.081 and P=0.090, respectively). Additionally, following the comparison of NVD and SC groups, no significant difference in the RNFLT between two groups was detected (P>0.05). According to IOP measurements in both eyes, IOP during delivery was significantly higher than before and after delivery in NVD group (P=0.05 and P=0.048, respectively) and CS group (P=0.028 and P=0.035, respectively). In addition, IOP in NVD group was higher than CS group in all states, but no statistically significant difference was seen among them (P>0.05). Conclusion: In general, the results showed that delivery type does not significantly affect RNFLT and IOP. It is also possible that an increase in IOP will lead to different results in women with pre-existing glaucoma or risk factors for it. Therefore, in cases with a positive background of glaucoma, specialized examinations and delivery under the supervision of a specialist are recommended.
