Anesthesia management with short acting agents for bilateral pheochromocytoma removal in a 12‐year‐old boy

Abstract: A 12-year-old boy with bilateral adrenal pheochromocytoma pretreated with furosemide, nifedipine, prazosin, and propranolol underwent surgical removal of the tumors. General anesthesia with desflurane, remifentanil infusion and thoracic epidural analgesia was performed. To control the blood pressure (BP), remifentanil up to 1 microg.kg(-1).min(-1) infusion rate, sodium nitroprusside, and esmolol infusions were administered successfully. Following the ligation of the adrenal veins, hemodynamic parameters were s… Show more

“…Although desflurane exerts sympathetic stimulation at an inspired concentration of 1.0-1.5 MAC [4], it is attenuated by a small dose of opioids [18]. Effective use of desflurane for controlling blood pressure during pheochromocytoma surgery and for rapid recovery from general anesthesia as a sole agent or in combination with remifentanil has been reported in a child as well as in adults [7][8][9].…”

“…Although desflurane elicits sympathetic activation, hypertension, and tachycardia when its inspired concentration exceeds 1.0 minimum alveolar concentration (MAC) [4], it provides faster and tighter hemodynamic control than other volatile agents at lower than 1.0 MAC [5,6], suggesting that desflurane might be safely used during anesthesia for resection of pheochromocytomas. However, there have been only a few reports describing anesthesia with desflurane in patients undergoing pheochromocytoma surgery [7][8][9]. We report the use of desflurane in a patient undergoing resection of extra-adrenal pheochromocytoma by laparotomy.…”

“…Esmolol has a ultra-short elimination half-life and it may be particularly advantageous due to this characteristic. In several studies, esmolol has been used successfully to control of hypertension in the patients with pheochromocytoma [8][9][10] . In our case, we could not decrease blood pressure with sodium nitroglycerine and we provided better hemodynamic stability with esmolol.…”

“…Although such crisis may be resolved by intraoperative phentolamine injections [2], agents of short action, like nitroprusside, landiolol and balanced general anesthesia using propofol and sevoflurane may improve hemodynamic management of phaeochromocytoma patients [18,21]. These agents could be applicable for immediate control of excessive blood pressure resulting not only from catecholamines secreted by phaeochromocytoma, but also due to the surgical manipulation of normal adrenal tissue and adrenal tumors [10].…”
