Abstract: The advent of new infusion pumps with pharmacokinetic models of remifentanil, sufentanil and propofol opens a new chapter in TIVA and aligns Brazil with the world tendency in TCI. Those systems will allow TCI of hypnotics and opioids concomitantly. However, the most important conclusion refers to the economy, since drugs used in those pumps will not be restricted to only one drug company, similar to what happened with propofol. Nowadays, TCI devices for the use of propofol and opioids, which accept any pharmac… Show more

“…Similar results can be obtained with total intravenous infusion devices that incorporate predicted plasma and effect site concentrations [16]. However, since we measure the volatile anaesthetic’s concentration in the lungs and only simulate the concentration of intravenous anaesthetic drugs, we do not know exactly and accurately what anaesthetic concentration is reaching the brain at all times throughout the surgical procedure.…”

“…A single intravenous injection easily induces a large drug level fluctuation in the blood which may lead to noticeable cardiovascular adverse reactions. By contrast, TCI may achieve a more accurate and stable blood drug level benefiting patient recovery (15). Remifentanil and sufentanil are applicable in TCI for the elderly (16,17).…”

“…In TCI, data are incorporated into a computer programmed system which works based on the parameters entered. [ 8 ] The Ce for induction has been defined, but recovery concentration may vary. The correlation between Ce at induction and recovery will enable to define recovery time and hence facilitate timely cessation of propofol infusion and thus early recovery.…”
