Zusammenfassung Hintergrund und Ziel der Studie Vor dem Hintergrund des Missverhältnisses zwischen Hornhautverfügbarkeit und -bedarf war das Ziel der Studie, signifikante Einflussfaktoren auf die Eignung sowie die Verwerfungsgründe von kornealem Spendergewebe zu eruieren und damit die Rate verworfener Hornhäute langfristig zu reduzieren. Patienten und Methoden 4140 Hornhäute von 2083 Spendern wurden zwischen 2006 und 2016 analysiert. Die untersuchten Einflussgrößen umfassten das Alter, das Geschlecht und die medizinische Vorgeschichte der Spender und Prozessierungsfaktoren wie die Post-mortem-Zeit. Die Daten wurden in eine Microsoft-Access-Datenbank übertragen und mittels logistischer Regressionsanalysen mit SPSS 20.0 statistisch ausgewertet. In einem multiplen Regressionsmodell wurden 6 Einflussfaktoren auf die Eignung des Spendergewebes analysiert. Alle Hornhäute, die nicht für eine elektive Keratoplastik infrage kamen, galten hierbei als ungeeignet. Ergebnisse Ein hohes Spenderalter > 80 Jahre erwies sich im Vergleich zur Referenzgruppe der Spender < 40 Jahre als signifikant negative Einflussgröße auf die Eignung der Hornhaut (p = 0,001). Auch eine vorangegangene Kataraktoperation minderte signifikant die Eignung (p < 0,001). Männliche Hornhäute waren signifikant weniger geeignet als weibliche Hornhäute (p = 0,028). Ein Hypertonus in der Spendervorgeschichte (p = 0,612), eine vorangegangene Chemotherapie (p = 0,745) oder ein Diabetes mellitus (p = 0,321) zeigten keinen Einfluss auf die Eignung. Sepsis (p = 0,026) und Multiorganversagen (MOV; p < 0,001) als Todesursachen des Spenders resultierten in signifikant höheren Verwerfungsraten aufgrund von Kontamination des Kulturmediums. Hornhäute, bei denen die Blutentnahme mehr als 12 h seit Versterben des Spenders zurücklag, mussten signifikant häufiger wegen einer positiven Spenderserologie verworfen werden (p < 0,001). Schlussfolgerungen Faktoren wie eine vorangegangene Kataraktoperation und ein hohes Spenderalter verringerten die Eignung des Spendergewebes. Ein Diabetes, ein Hypertonus und eine Chemotherapie in der Spendervorgeschichte zeigten keinen statistisch signifikanten Einfluss auf die Eignung. Todesursachen wie Sepsis und Multiorganversagen erhöhten die Kontaminationsrate des Kulturmediums. Eine postmortale Blutentnahme nach mehr als 12 h geht mit einem erhöhten Risiko für ein positives Serologieergebnis einher.

Background: To assess the corneal structure after femtosecond laser small-incision lenticule extraction (SMILE) in diabetic rabbits with different blood glucose levels.Methods: Twelve diabetic rabbits were randomly distributed into three groups based on blood glucose levels: group A (G-A), 16-20 mmol/L (n=4); group B (G-B), 4.33-8.61 mmol/L (n=4); and group C (G-C), healthy controls (n=4). After 2 weeks, SMILE was performed. Corneas were evaluated using anterior segment optical coherence tomography (AS-OCT) and in vivo confocal microscopy (IVCM) preoperatively on day 1 and postoperatively at weeks 1, 4, and 12.Results: Morphological results: Twelve weeks after surgery, a corneal interlamellar stromal scar in three eyes and corneal stroma edema in one eye were found in G-A; no obvious scar edema occurred in G-B and G-C. In G-A, the number of abnormal cells in each corneal layer increased, the cell arrangement was irregular, and nerve fibers decreased significantly; the morphology and arrangement in G-B and G-C were regular. Cell density results: Actual central corneal thickness (CCT) after SMILE was thicker than theoretical CCT in each group (P<0.05). The corneal thickness deviation (△CT) of G-A was thicker than that of G-B and G-C, and the pre-keratocyte density (pre-KD) of G-A was lower than that of G-B and G-C (P<0.05). Endothelial cell density (ECD) at 4 and 12 weeks after surgery only in G-A was lower than that before surgery.Conclusions: Hyperglycemia is an adverse factor affecting corneal structure after SMILE. SMILE is safe and effective for diabetic rabbits with good blood glucose control.

ImportanceThere is limited literature on paediatric donors in endothelial keratoplasty.BackgroundThis study investigated the efficacy of and appropriate paediatric donor age for Descemet’s stripping endothelial keratoplasty (DSEK).DesignRetrospective and observational case series.ParticipantsThirty‐eight consecutive patients underwent DSEK with paediatric donor corneas.MethodsThe age of the donors ranged from 32 weeks gestation (premature neonate) to 3 years old. All donor consents were obtained from the parents. The causes of donor death included traffic accident, congenital heart disease and neonatal respiratory distress syndrome.Main Outcome MeasuresThe outcome measures included best‐corrected visual acuity, endothelial cell loss and complications.ResultsBest‐corrected visual acuity at last follow‐up was >20/40 in 28 of 38 eyes (73.7%). The mean preoperative endothelial cell density of donor corneas was 4682 ± 520 cells/mm2. The mean endothelial cell density of grafts was 3977 ± 556 cells/mm2 at 18 months postoperatively. Three lenticules from premature neonate donors exhibited severe contraction postoperatively. The edge of six lenticules from donors <1‐year‐old exhibited contraction in the early postoperative period and gradually flattened spontaneously. Graft detachment occurred in one patient.Conclusions and RelevanceDSEK with paediatric donor corneas can achieve good clinical outcomes. The corneal lenticules from 1‐ to 3‐year‐ old donors are suitable for DSEK while those from donors <1‐year‐old are less suitable due to the possibility of severe postoperative graft contraction.

Abstract: Age, race, ocular surgery (cataract and refractive), and DM seem to significantly affect donor corneal ECD. Of these variables, age, a history of cataract surgery, and DM were found to be the greatest risk factors for inadequate donor cell density (less than 2000/mm).
