Information infrastructures involve the notion of a shared, open infrastructure, constituting a space where people, organizations, and technical components associate to develop an activity. The current infrastructure for medical image sharing, based on PACS/DICOM technologies, does not constitute an information infrastructure since it is limited in its ability to share in a scalable, comprehensive, and secure manner. This paper proposes the DICOMFlow, a decentralized, distributed infrastructure model that aims to foment the formation of an information infrastructure in order to share medical images and teleradiology. As an installed base, it uses the PACS/DICOM infrastructure of radiology departments and the internet e-mail infrastructure. Experiments performed in real and simulated environments have indicated the feasibility of the proposed infrastructure to foment the formation of an information infrastructure for medical image sharing and teleradiology.Keywords Digital imaging and communications in medicine (DICOM) . Recently, the cloud service has the potential to replace conventional cluster and grid systems. The objective of migrating apps to the cloud is to minimize maintenance and procurement expenses while simultaneously boosting scalability and availability. However, embra=cing cloud technology created some challenges, such as the complexity of cloud storage. In addition, many clients underestimate if it is not plug-and-play. Each vendor has its access methods, and nonstandard application programming interfaces (APIs) make integrated applications, such as archiving or sharing data with cloud storage, complicated, costly, and require high throughput. Furthermore, organizations did not have many alternatives for implementing high-performance object storage systems in the cloud and on-premises data centers until now. In this paper, we would like to suggest a storage gateway as a solution to this issue and will optimize it using Transfer Acceleration and Diff algorithms to improve the performance, Intelligent Tiering to reduce costs, and Server-Side encryption for extra protection. Moreover, utilizing Storage Gateway has proven can provide more efficient integration between the on-premises data center environment and the AWS Cloud Storage ecosystem that is safer and more reliable. This technology can work in a common data center environment regardless of the vendor used by the company it can communicate seamlessly with the AWS Environment.

Background and Objective: Nowadays usage paradigms of medical imaging resources are requesting vendor-neutral archives, accessible through standard interfaces, with multi-repository support. Regional repositories shared by distinct institutions, teleradiology as a service at Cloud, teaching and research archives, are illustrative examples of this new reality. However, traditional production environments have a server archive instance per functional domain where every registered client application has access to all studies. This paper proposes an innovator ownership concept and access control mechanisms that provide a multi-repository environment and integrates well with standard protocols.Methods: A secure accounting mechanism for medical imaging repositories were designed and instantiated as an extension of a well-known open-source archive. A new Web services layer was implemented to provide a vendor-neutral solution complaint with modern DICOM-Web protocols for storage, search and retrieve of medical imaging data.Results: The concept validation was done through the integration of proposed architecture in an open-source solution. A quantitative assessment was performed for evaluating the impact of the mechanism in the usual DICOM Web operations.Conclusions: This article proposes a secure accounting architecture able to easily convert a standard medical imaging archive server in a multi-repository solution. The proposal validation was done through a set of tests that demonstrated its robustness and usage feasibility in a production environment. The proposed system offers new services, fundamental in a new era of Cloud-based operations, with acceptable temporal costs.
