Abstract: Aims To investigate if extra virgin olive oil (EVOO) or palm oil enriched chocolate spreads consumption leads to different results in terms of plasma ceramides concentration, glucose and lipid metabolism, inflammatory markers and appetite regulation in young healthy subjects. Methods In a 2‐week, double‐blind, cross‐over, randomised controlled trial, 20 healthy, normal‐weight subjects with a mean age of 24.2 years (SD: 1.2), consumed chocolate spread snacks (73% of energy [%E] from fat, 20% from carbohydrates … Show more

“…The characteristics of the included studies are shown in Table 1. Among the 52 studies selected, 44 7,17–21,28–66 are randomized clinical trials with a parallel design and eight 67–74 have a crossover design. The samples of the included studies ranged from 23 to 351 participants, with a mean age ranging from 23 to 79 years.…”

“…As far as lipid replacement RCTs are concerned, an Italian RCT [ 186 ] also examined the effect of EVOO against palm oil in improving ceramide concentrations. The results revealed that partial replacement of SFAs with MUFAs in the form of a chocolate-spread integrated in a isocaloric diet reduced the detrimental effects of SFAs on insulin sensitivity and reduced circulating harmful sphingolipids in young adults [ 186 ].…”

“…Alternatively, in subjects with obesity, overfeeding saturated fatty acids for three to eight weeks resulted in elevated IHTG and plasma CER concentrations while overfeeding unsaturated fat or carbohydrates did not (194,201). These findings were recently confirmed in healthy subjects during a short-term (two-week) study demonstrating a diet enriched with saturated fat (when compared to unsaturated fat) increased plasma CER and HOMA-IR (200). Interestingly, calorie restriction following saturated fat overfeeding reduced the adverse metabolic effects of the diet, including a reduction of CER concentrations to the level of control subjects (201).…”

“…Interestingly, calorie restriction following saturated fat overfeeding reduced the adverse metabolic effects of the diet, including a reduction of CER concentrations to the level of control subjects (201). Most (194,198,(200)(201)(202), but not all (199,202), studies support dietary modifications positively impacting plasma CERs through reductions in total CERs and specific lipotoxic species. No studies in humans have investigated CER concentrations directly in liver tissue in a setting of dietary modifications although many studies in rodent models have (149,167,207).…”
