Alveolar and cystic echinococcosis: towards novel chemotherapeutical treatment options

Abstract: Echinococcus granulosus and Echinococcus multilocularis are cestode parasites, of which the metacestode (larval) stages cause the neglected diseases cystic echinococcosis (CE) and alveolar echinococcosis (AE), respectively. The benzimidazoles albendazole and mebendazole are presently used for the chemotherapeutical treatment, alone or prior to and after surgery. However, in AE these benzimidazoles do not appear to be parasiticidal in vivo. In addition, failures in drug treatments as well as the occurrence of s… Show more

“…A number of compounds have been investigated in the past as treatment alternatives for AE, using in vitro-cultured parasites and/or in vivo rodent models. These include other benzimidazole derivatives, broad-spectrum anti-infective agents, and anticancer drugs (reviewed in references [18][19][20]. However, none of the compounds investigated has been translated into clinical application, either due to low efficacy, high toxicity, or simply because the pharmaceutical industry has not been prepared to invest in a disease without a meaningful market return (21).…”

“…However, therapy using these drugs does not result in parasiticidal activity in vivo or in vitro, and treatment failures and/or the occurrence of adverse effects have been reported, leading to discontinuation of treatment or to progressive disease. Therefore, the search for novel drug candidates for the treatment of AE has to be pursued (7).…”

“…Failures in drug treatment and/or the occurrence of side effects has been reported, leading to discontinuation of treatment or to progressive disease. Due to this unsatisfactory situation, novel drug candidates for the treatment of AE need to be identified (17,18).…”
