All-arthroscopic AMIC procedure for repair of cartilage defects of the knee

Abstract: PurposeBone marrow mesenchymal stem cells were introduced into clinical practice due to their ability to differentiate into many types of cells. Autologous matrix-induced chondrogenesis (AMIC) combines the microfracture method with matrix-based techniques that utilizes a collagen membrane to serve as a scaffold for new bone marrow mesenchymal stem cells, allowing effective reconstruction of even large fragments of a damaged cartilage surface.MethodsAll-arthroscopic technique to repair knee cartilage defects us… Show more

“…A mandatory prerequisite for such a procedure is equilibration of the joint pressure with atmospheric pressure. 5 Substituting gas for saline solution is an alternative to create a fluidfree environment but has been criticized because of the increased risk of gas-induced vascular embolization. Another issue is the constant outflow of gas, which complicates the insertion of the matrix.…”

“…5 Autologous chondrocyte implant techniques are effective treatments for chondral and osteochondral lesions, 15 but the use of these matrixes in a single procedure has facilitated the arthroscopic application of both the MACI 6,7,9 and AMIC techniques. 10 There are very few reports on the treatment of patellar lesions using these techniques arthroscopically. 12,13,16 The main difficulty when dealing with patellar chondral lesions arthroscopically is the inability to approach the articular surface perpendicularly.…”

“…9 Both techniques have been used for the treatment of femoral condylar [6][7][8][9] or, less frequently, tibial plateau lesions. 10 There are very few reports on the treatment of patellar lesions using these techniques arthroscopically. [11][12][13] We describe an arthroscopic approach to the patella with the patient lying in the supine position.…”
