Alcohol Drinking in Never Users of Tobacco, Cigarette Smoking in Never Drinkers, and the Risk of Head and Neck Cancer: Pooled Analysis in the International Head and Neck Cancer Epidemiology Consortium

Abstract: Our results represent the most precise estimates available of the independent association of each of the two main risk factors of head and neck cancer, and they exemplify the strengths of large-scale consortia in cancer epidemiology.

“…13 The results in the present study for the effect of tobacco smoking and alcohol consumption confi rm what is long known, that these two exposures are the main risks for oral cavity and oropharynx cancers. 10 No association was seen between complete dental prosthesis and oral cancer, corroborating other studies. 8,11,12 On the other hand, high association of frequent gum bleeding during toothbrushing and oral cancer was found.…”

“…Linear trend was estimated for alcohol consumption and smoking by fi tting the categorical variable into the logistic regression model as a continuous variable. 10 The study was approved by the Research Ethical Committees of all participating medical sites and Universidade de São Paulo School of Public Health, as well as by the National Human Research Ethics Committee of the Brazilian Ministry of Health. Cases and controls were informed of the objectives and uses of the research project both in person and through a consent form, which was signed by them.…”

“…Alcohol consumption has been linked to periodontitis 19 and tooth wear 20 and both smoking and alcohol are closely linked to the incidence of oral cancer. 21 The incidence of oral cancer is also strongly related to social and economic deprivation, 22 particularly for men, and this widening inequalities gap makes it all the more important that all avenues to address possible risk factors are explored. All patient examinations in the CDS include smoking and alcohol cessation advice related to risks of oral disease and cancer.…”

“…19 The reported pooled cancer risk estimate is 3.43 times higher in smokers compared with non-smokers (95% CI, 2.37 -4.94). 20 Yet in this study smoking did not figure prominently in patients' accounts of why they contracted the disease.…”
