Permanent repository link ABSTRACTOngoing advances in technology are increasing the scope for enhancing and supporting older adults' daily living. The digital divide between older and younger adults raises concerns, however, about the suitability of technological solutions for older adults, especially for those with impairments. Taking older adults with Age-Related Macular Degeneration (AMD) as a case study, we used user-centred and participatory design approaches to develop an assistive mobile app for self-monitoring their intake of food [12,13]. In this paper we report on findings of a longitudinal field evaluation of our app that was conducted to investigate how it was received and adopted by older adults with AMD and its impact on their lives. Demonstrating the benefit of applying inclusive design methods for technology for older adults, our findings reveal how the use of the app raises participants' awareness and facilitates self-monitoring of diet, encourages positive (diet) behaviour change, and encourages learning.

Aim: To investigate the association of diet and other modifiable risk factors with the prevalence of age-related macular degeneration (ARMD) in rural and urban communities of a Mediterranean population in the northeast of Italy. Methods: Across-sectional population-based study was conducted among subjects aged over 60 years. A food frequency questionnaire (FFQ) was used to assess the consumption of different food categories, i.e., protective (P), risky (R), lutein-rich (L) and neutral (N). Smoking habit and alcohol intake were also examined. Macular pigment was measured by Raman spectroscopy. Results: P food intake reduced the risk of large drusen (ARM2; OR 0.93; 95% CI 0.89-0.96) within the rural community. In this sub-group, R foods resulted in a slight association with large drusen, though the R/P food ratio was highly correlated with ARM2 (OR 1.21; 95% CI 1.12-1.31). Raman measures showed an age-dependent decrease but did not correlate with lutein intake. Smoking habit showed a positive association with ARM2 among women (OR 2.40; 95% CI 1.54-3.75), whereas alcohol consumption resulted in protective odds (OR 0.72; 95% CI 0.60-0.86). Conclusion: FFQ analysis confirmed the role of P and R foods and the benefit of a Mediterranean diet in ARMD. Moderate alcohol consumption showed a beneficial effect, whereas the deleterious role of a smoking habit was more evident in females.

A szem struktúráit vérrel ellátó érendothelium működészavarának kulcsfontosságú a szerepe az időskori maculadegeneráció létrejöttében, és indirekt bizonyítékok arra utalnak, hogy az időskori maculadegeneráció érbetegség, végső soron az egész érrendszer megbetegedésének a része. Az ártalmak, a rizikófaktorok oxidatív stresszhez vezetnek, ami aktiválja az endothelt, és szövetkárosodás alakul ki. Az erek, így a szem érhártyájában, a chorioideában lévő erek falát is számos ismétlődő és/vagy elhúzódó inger éri, amelyek mechanikai, fi zikai, kémiai, mikrobiológiai, immunológiai, illetve genetikai eredetűek lehetnek. Bármilyen hosszan tartó/ismétlődő ártalom, amely működésbe hozza a védekezés láncreakcióját, gyulladást vált ki. Az ártalmas behatás eliminálásának sikertelensége endothelműködés-zavart, ér/szö-vet károsodást, időskori maculadegenerációt eredményez(het). Mindezek a tények arra utalnak, hogy az időskori maculadegeneráció érbetegség, az egész érrendszer megbetegedésének a része. As a consequence, pathological changes resulting in vascular injury (e. g. atherosclerosis, age-related macular degeneration) may be evolved. Risk factors can also act directly on the endothelium through an increased production of reactive oxygen species promoting an endothelial activation, which leads to endothelial dysfunction, the onset of vascular disease. Thus, endothelial dysfunction is a link between the harmful stimulus and vascular injury; any kind of harmful stimuli may trigger the defensive chain that results in infl ammation that may lead to vascular injury.
