Anemia at Presentation Predicts Acute Mortality and Need for Readmission Following Geriatric Hip Fracture

Background: There is a paucity of research regarding the relationship between anemia and postoperative morbidity and mortality among geriatric patients presenting with hip fracture. The objective of this study was to determine the effect of anemia at presentation on 30-day morbidity and mortality among geriatric patients with hip fracture. Methods: The American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database was queried for all hip fracture patients ≥60 years old from 2011 to 2016. Included were all emergency unilateral, nonpathological hip fractures (femoral neck, intertrochanteric, or subtrochanteric) treated with arthroplasty, intramedullary nailing, or open reduction and internal fixation. Anemia was classified as a hematocrit (HCT) level of <0.41 and <0.36 for male and female patients, respectively. Age, body mass index (BMI), race, comorbidities, smoking status, American Society of Anesthesiologists (ASA) class, baseline functional status, time to surgery, operative time, anesthesia type, need for transfusion, fixation method, length of stay (LOS), and discharge destination were collected. Our primary outcome of interest was 30-day postoperative mortality, with all-cause readmission and any postoperative ischemic events (cerebrovascular accident [CVA] and myocardial infarction [MI]) analyzed as secondary outcomes. A multivariable regression analysis was performed and odds ratios (ORs) with 95% confidence intervals (CIs) were calculated while controlling for confounding variables. Results: Of 34,805 patients identified, 22,469 (65%) were anemic at presentation (63% female; mean age, 80 ± 8 years), while 12,336 (35%) were non-anemic (85% female; mean age, 79 ± 8 years). Anemia at presentation was independently associated with higher odds of mortality (OR,1.3 [95% CI, 1.1 to 1.5]) and readmission (OR, 1.2 [95% CI, 1.1 to 1.3]), while no relationship was observed for MI (OR, 1.1 [95% CI, 0.9 to 1.4]) or CVA (OR, 0.8 [95% CI, 0.6 to 1.1]). Conclusions: Our findings suggest that anemia at presentation is associated with greater 30-day postoperative morbidity and mortality in geriatric hip fracture patients. Additional research should focus on elucidating this modifiable risk factor and advancing the preoperative optimization of hip fracture patients. Level of Evidence: Prognostic Level IV . A análise da série eritrocitária realizada a partir do hemograma fornece informações importantes sobre os índices hematimétricos encontrados na composição sanguínea, assim é possível verificar o real estado hematológico do paciente. Objetivo: Correlacionar a dosagem de hemoglobina com os demais índices hematimátricos, com intuito de identificar a relação entre os mesmos e o estado anêmico. Procedimentos: Foram analisados 2023 resultados de hemograma de pacientes atendidos no Laboratório de Análises Clínicas da Universidade Evangélica de Goiás, situada na cidade de Anápolis/GO durante o período 01/01/2017 a 18/12/2017. Para as análises estatísticas, utilizou-se o programa Epi Info 3.5.1 e o teste χ 2 adotando o intervalo de confiança de 95% para aceitação de diferenças nos testes estatísticos e um p-value < 0,05. Resultados e Discussão: O processo anêmico foi visualizado em 417 do total dos laudos estudados, podendo verificar que as reduções das taxas de Hb estiveram fortemente relacionadas com a diminuição do número absoluto de eritrócitos, VCM, HCM e elevação do RDW. Conclusão: Deste modo, como a determinação de hemoglobina esteve correlacionada aos índices hematimétricos, esses achados podem ser facilitadores na clínica para a descoberta da casuística e tratamento de anemia.

Objective: To estimate the prevalence of anaemia in Brazilian children up to 83·9 months old. Design: Systematic review and meta-analysis, using databases PubMed, Scopus, SciELO, Lilacs, Google Scholar, Periódicos Capes, Arca, Biblioteca Virtual em Saúde, Microsoft Academic Search and Cochrane Library using search terms: anaemia, prevalence, child and Brazil. PROSPERO Registration number: CRD42020208818. Setting: Cross-sectional, cohort, case–control and intervention studies published between 2007 and 2020 were searched, excluding those who assessed children with an illness or chronic condition. The main outcome was anaemia prevalence. Random effects models based on the inverse variance method were used to estimate pooled prevalence measures. Sensitivity analyses removed studies with high contribution to overall heterogeneity. Participants: From 6790 first screened, 134 eligible studies were included, totalling 46 978 children aged zero to 83·9 months analysed, with adequate regions representativeness. Results: Pooled prevalence of anaemia was 33 % (95 % CI 30, 35). Sensitivity analyses showed that withdrawal of studies that contributed to high heterogeneity did not influence national average prevalence. Conclusions: Childhood anaemia is still a serious public health problem in Brazil, exposing 33 % of Brazilian children to the anaemia repercussions. The main limitation of the study is the estimation of national prevalence based on local surveys, but a large number of studies were included, with representation in all regions of the country, giving strength to the results. In Brazil, more public policies are needed to promote supplementation, fortification and access to healthy eating to reduce the high level of anaemia among children.
