Adverse tissue reaction to corrosion at the neck-stem junction after modular primary total hip arthroplasty

Abstract: Complications related to the neck-stem junction of modular stems used for total hip arthroplasty (THA) are generating increasing concern. A 74-year-old male had increasing pain and a cutaneous reaction around the scar 1 year after THA with a modular neck-stem. Imaging revealed osteolysis of the calcar and a pseudo-tumour adjacent to the neck-stem junction. Serum cobalt levels were elevated. Revision surgery to exchange the stem and liner and to resect the pseudo-tumour was performed. Analysis of the stem by sc… Show more

“…30,32,33,41,42 Relative motion is an indicator for the strength of the taper interlock. 43 Reduced relative motion can contribute to a reduction of wear and corrosion potential. 26 Although forces during cyclic loading (F max = 4300 N) were higher than two of the assembly force groups, the difference in seating depth between the assembly force groups was still present after 2000 loading cycles, indicating that cyclic loading does not provide similar seating as higher assembly forces within the amount of loading cycles used in this experiment.…”

“…Passivity is the most common situation under the operating conditions of the alloy in the human body (analysis of retrievals showed there is an oxide film on the explants [68]). It is defined as the electrochemical situation where the formation of a 2–10 nm thin oxide film takes place.…”

“…The modular stem also yields concerns regarding taper junction corrosion. 20 THA with trochanteric osteotomy is another option for THA with severe LLD. 21 With trochanteric osteotomy, exposure of the acetabulum could be facilitated.…”
