Adoption, Reach, Implementation, and Maintenance of a Behavioral and Mental Health Assessment in Primary Care

Abstract: PURPOSE Guidelines recommend screening patients for unhealthy behaviors and mental health concerns. Health risk assessments can systematically identify patient needs and trigger care.
