Achieving the 95-95-95 UNAIDS targets requires meeting the needs of adolescents, however we lack evidenced-based approaches to improving adolescent adherence to antiretroviral therapy (ART), increasing viral suppression, and supporting general wellbeing. We developed Family Connections as a group intervention for adolescents and their adult caregivers and conducted a randomized controlled trial in Ndola, Zambia to test feasibility and acceptability. Fifty pairs (n = 100) of adolescents (15-19 years and on ART ≥ 6 months) and their caregivers were randomly assigned either to the intervention consisting of 10 group sessions over 6 months, or to a comparison group, which received the usual care. Each pair completed baseline and endline surveys, with adolescents also undergoing viral load testing. Of the 24-intervention adolescent/ caregiver pairs, 88% attended at least eight group sessions. Most adolescents (96%) and all caregivers would recommend Family Connections to peers. Adolescent viral failure decreased but did not significantly differ by study group. Adolescents in the intervention group showed a greater reduction in HIV-related feelings of worthlessness and shame than the comparison group. The feasibility, acceptability, and the positive trend toward significantly reducing internalized stigma, generated by this Family Connections pilot study, contributes valuable data to support adolescent/caregiver approaches that use peer groups.

Gençlik dönemi sağlık alışkanlıklarının ve cinsel davranışların şekillenmeye başladığı, erişkin dönem yaşam tarzının belirlendiği özel bir dönemdir. [1-5] Bu dönemde ABSTRACT OBJECTIVES: This study was carried out in order to determine the opinions and attitudes of midwifery students on non-marital sexual intercourse. MATERIAL and METHODS: The descriptive research was carried out with the students of midwifery department of the Faculty of Health Sciences of Amasya University in 2017-2018 academic year/spring semester (N=86). The data were collected by using two forms which were prepared by the researchers by using the literature and evaluating their opinions and attitudes about the socio-demographic characteristics of the students and out-of-marriage sexual intercourse and analyzed in SPSS package program. RESULTS: The mean age of the students was 20.08±2.18 years. It was determined that 68.6% of the students had sufficient information about sexual intercourse, 58.14% of the students got written information from the related information, 79.07% of them could not speak comfortably with their family and 87.2% stated that their religious beliefs may prevent them from having sexual experiences. There was a significant difference between the knowledge levels of students about sex and their class (p=0.000): 43.33% of the first class, 65% of the second class, 83.33% of the third class and 100% of the fourth class have enough knowledge about sexual intercourse. CONCLUSION: Students can be strengthened in matters such as deciding on relevant issues, taking responsibility for their decisions, acting professionally with the problems they are facing, receiving support for their needs, and affecting the value of the social value judgments. Sexual health related needs should be identified and trainings should be given to the subjects that students need.

Health‐related needs reported by adolescents living with HIV and receiving antiretroviral therapy in sub‐Saharan Africa: a systematic literature review

Introduction Adolescents living with HIV (ALHIV) on antiretroviral therapy (ART) have specific health needs that can be challenging to deliver. Sub‐Saharan Africa (SSA) is home to 84% of the global population of ALHIV, of whom about 59% receive ART. Several studies in SSA have demonstrated health service gaps due to lack of synchronized healthcare for ALHIV receiving ART. We conducted a systematic review of health‐related needs among ALHIV on ART in SSA to inform decisions and policies on care. Methods We searched MEDLINE, Web of Science, EMBASE, PsycINFO, Cochrane library and grey literature for studies reporting health‐related needs among ALHIV receiving ART in SSA, between January 2003 and May 2020. Results and discussion Of the 2333 potentially eligible articles identified, 32 were eligible. Eligible studies were published between 2008 and 2019, in 11 countries: Zambia (7), Uganda (6), Tanzania (4), South Africa (4), Kenya (3), Ghana (2), Zimbabwe (2), Rwanda (1), Malawi (1), Botswana (1) and Democratic Republic of Congo (1). Seven categories of health needs among ALHIV were identified. In descending order of occurrence, these were: psychosocial needs (stigma reduction, disclosure and privacy support, and difficulty accepting diagnosis); dependency of care (need for family and provider support, and desire for autonomy); self‐management needs (desire for better coping strategies, medication adherence support and reduced ART side effects); non‐responsive health services (non‐adolescent friendly facility services and non‐compatible school system); need for food, financial and material support; inadequate information about HIV (desire for more knowledge to fight misinformation and misconception); and developmental and growth needs (desire to experience sex, parenthood and love). Ecological analysis identified different priority needs between ALHIV, their caregivers and healthcare providers, including psychosocial needs, financial challenges and non‐responsive health services, respectively. Conclusions To respond effectively to the health needs of ALHIV and improve ART adherence, interventions should focus on stigma reduction, disclosure challenges and innovative coping mechanisms for ART. Interventions that address the health needs of ALHIV from the perspective of carers and providers, such as financial support schemes and adolescent‐friendly healthcare strategies, should supplement efforts to improve adolescent ART adherence outcomes.
