This study was designed to evaluate the prevalence of cardiometabolic comorbidities and the changes in left ventricular geometry and function in 135 subjects subgrouped according to low or normal total adiponectin plasma (ADPN) levels. Left ventricular (LV) internal diameter/height, total LV mass (LVM) and LVM index (LVMI), relative wall thickness (RWT), LV ejection fraction by echocardiography and diastolic parameters by pulsed-wave Doppler were calculated. Body mass index (BMI) (p \ 0.0001), waist-to-hip ratio (p \ 0.03), triglycerides (p \ 0,001), prevalence of obesity (p \ 0.005), visceral obesity (p \ 0.003), left ventricular hypertrophy (LVH) (p \ 0.001), metabolic syndrome (p \ 0.0003) and coronary artery disease (CAD) (p \ 0.003) were significantly increased and high-density lipoprotein-cholesterol (p \ 0.001) was significantly reduced in hypo-ADPN than normal-ADPN subjects. LVM, LVMI, interventricular septum thickness and RWT were significantly (p \ 0.0001) higher and left ventricular ejection fraction was significantly (p \ 0.0002) lower in hypo-ADPN than normal-ADPN patients. LVMI correlated directly with BMI (p \ 0.001), mean blood pressure (p \ 0.001), metabolic syndrome (MetS) (p \ 0.001) and inversely with ADPN (p \ 0.0001). The prevalence of LVH (p \ 0.001) and CAD (p \ 0.01) was higher in subjects with normal-ADPN and MetS, while the presence of MetS did not change this finding in hypo ADPN group. Both models of regression analysis indicated that ADPN and BMI resulted independently associated with LVMI. In conclusion, our data seem to indicate that hypoadiponectinemia might be associated with an increased prevalence both of clinical comorbidities and increased LVMI. In this subset of subjects, ADPN and BMI, more than MetS, are able to explain cardiac damage. Accordingly, ADPN might become a new target in the management of cardiometabolic risk.

A bstract Background/O bjective: Most of the interventional studies have investigated the impact of the diet on adiponectin and leptin concentrations only in men or in women. Consequently, it is still unknown whether the consumption of a healthy diet influences in a sex-specific manner these adipocytokines.We examined sex differences in the effects of the Mediterranean diet (MedDiet) on adiponectin and leptin concentrations and determined whether changes in these adipocytokines are associated with changes in cardiovascular risk factors in both sexes.Subjects/Methods: Participants were 38 men and 32 premenopausal women (24-53y) with slightly elevated LDL-C concentrations (3.4-4.9 mmol/l) or total cholesterol/HDL-C and cardiovascular risk factors were measured before and after a 4-week fully-controlled isoenergetic MedDiet.Results: Adiponectin concentration decreased in response to the MedDiet, but this decrease reached statistical significance only in men (P<0.001 for men and P=0.260 for women; sex-by-time interaction, P=0.072). Adjustments for body weight or waist circumference did not change results obtained.Changes in adiponectin were positively associated with concomitant variations in HDL-C in men (r=0.52, P=0.003) and with variations in apo A-1 and insulin sensitivity as calculated by both HOMA-IS and Cederholm indices in women (respectively r=0.44, P=0.021; r=0.79, P<0.001 and r=0.47, P=0.020). The MedDiet had no impact on leptin and leptin-to-adiponectin ratio in both sexes.Conclusions: Results suggest sex difference in adiponectin response to the short-term consumption of the MedDiet, with only men experiencing a decrease. Also sex-specific patterns of associations between changes in adiponectin concentration and changes in cardiovascular risk factors were observed.

ÖzetAmaç: Psoriazis patogenezi tam olarak anlaşılamamış kronik inflamatuvar bir hastalıktır. Adipositlerden üretilen adiponektinin (AN) antidiyabetik, antiaterojenik ve antiinflamatuvar etkileri olduğu gösterilmiştir. Son yıllarda yapılan çalışmalarda metabolik sendrom düşük AN düzeyi ile ilişkilendirilmiştir. Literatürde az sayıda çalışmada psoriaziste AN düzeyi araştırılmıştır. Çalışmamızda psoriazisli olgularda AN düzeyini ve hastalık şiddeti ile ilişkisini araştırmayı amaçladık. Gereç ve Yöntem: Çalışmamıza 46 psoriazis vulgarisli hasta ile yaş ve cinsiyet uyumlu 40 sağlıklı kontrol grubu dahil edildi. Hastaların dermatolojik muayeneleri yapıldı, psoriazis alan ve şiddet indeksi (PAŞİ) hesaplandı. Her 2 grupta demografik özellikler sorgulandı ve vücut kitle indeksi belirlendi. Serum AN ve CRP düzeyleri her iki grupta ölçüldü ve istatistiksel olarak karşılaştırıldı. Bulgular: Ortalama serum AN düzeyleri hasta grubunda kontrol grubuna göre daha düşük saptandı; ancak istatistiksel olarak anlamlı değildi. Serum AN düzeyi yaş, hastalık başlangıç yaşı ve hastalık süresi ile ilişkili bulunmadı. Psoriazis hastalarında CRP arttıkça, serum AN düzeyinin düştüğü tesbit edildi. PAŞİ arttıkça AN düzeyinde istatistiki olarak anlamlı azalma gözlendi. PAŞİ ile vücut kitle indeksi arasında istatistiki bir ilişki gözlenmedi. Sonuç: Psoriaziste AN düzeyi, metabolik sendrom ile birlikteliğinin bir göstergesi olabilir. Sum maryBackground and Design: Psoriasis is a chronic inflammatory disease in which the pathogenesis has not been completely understood yet. Adiponectin (AN) produced by adipocytes has antidiabetic, antiatherogenic and anti-inflammatory effects. Recent studies demonstrated that metabolic syndrome is related with low levels of AN. In the literature, there have been a few studies investigating AN levels in psoriasis. In this study, we purposed to investigate AN levels in patients with psoriasis. Materials and Methods: Forty-six patients with psoriasis and 40 age-and sex-matched healthy controls were included in our study. Dermatological examinations were performed and psoriasis area severity index (PASI) was calculated. In both groups, demographic features were questioned and body mass index were defined. Serum AN and C-reactive protein (CRP) levels were determined in both groups and were compared statistically. Results: The mean serum AN levels in patients were lower than in controls, but no statistically significant difference was detected between the groups.
