This in vitro study compared the shear bond strength (SBS) and adhesive remnant index (ARI) of two systems for bonding orthodontic brackets to enamel. The first system involved a self-etching primer (Beauty Ortho Bond, BO) containing surface pre-reacted glass filler. The second involved a primer applied with phosphoric acid etching (Transbond XT, TX). Ninety-six extracted human premolars were divided into eight groups: Group I (TX/direct bonding), Group II (TX/indirect bonding), Group III (BO/direct bonding), and Group IV (BO/indirect bonding). Groups V-VIII were identical to Groups I-IV, respectively, but were also subjected to 1,500 thermal cycles between 5 and 55°C. ARI was scored by binocular microscopy. SBS was analyzed by three-way ANOVA and the Bonferroni test. ARI was analyzed by the chi-squared test. The BO groups showed lower SBS and ARI results than the TX groups. SBS was significantly influenced by the primer material, bonding technique, and thermal cycling.

ÖZETSabit ortodontik elemanların, bantlara gereksinim olmaksızın doğrudan dişler üzerine yapıştırıldığı 'Direkt Yapış-tırma Tekniği' ile braketleme için klinikte harcanan süre kısaltılmış, daha estetik ve hijyenik bir ortodontik tedavi uygulama imkanı elde edilmiştir. Ancak bu teknikte, özel-likle arka dişlere doğrudan görüş ve ulaşım sorunu gibi sebeplerle braket konumlarında hatalarla karşılaşılmıştır. Bu hataların önlenmesi amacıyla 'İndirekt Yapıştırma Tekniği' geliştirilmiştir. Bu tekniğin ilk geliştirildiği yıllarda gözlenen direkt yapıştırılan braketlere kıyasla bağlanma dayanımının daha az olması, yüksek braket kopma oranları, braketlerin etrafından taşan adezivlerin periodontal dokuları irrite etmesi ve oral hijyeni olumsuz etkilemesi, laboratuvar aşamasının uzun ve zaman alıcı olması gibi pek çok olumsuz özelliği günümüzde geliştirilen materyaller ve tekniklerle birlikte elimine edilmiştir. Günümüzde indirekt teknik ile yapıştırılan braketler, direkt yapıştırılan braketlerle benzer bağlanma dayanımı göstermektedir. Ayrıca indirekt ve direkt yapıştırma tekniklerinin, periodontal dokular üzerine etkileri de benzerdir. Ancak indirekt yapıştırma tekniği, hem laboratuvar hem de klinik aşamalarında daha fazla özen ve hassasiyet gerektirmektedir ve maliyeti daha yüksektir. Hekimin bu iki yapıştırma yönteminden hangisini seçeceği; laboratuvar süresi, klinik süresi, maliyet, hasta konforu gibi etkenler ve kendi kişisel tercihine bağlı olarak değişebilir.

A photopolymerized composite hydrogel and surgical implanting tool for a nucleus pulposus replacement

a b s t r a c tNucleus pulposus replacements have been subjected to highly controversial discussions over the last 40 years. Their use has not yet resulted in a positive outcome to treat herniated disc or degenerated disc disease. The main reason is that not a single implant or tissue replacement was able to withstand the loads within an intervertebral disc. Here, we report on the development of a photo-polymerizable poly(ethylene glycol)dimethacrylate nano-fibrillated cellulose composite hydrogel which was tuned according to native tissue properties. Using a customized minimally-invasive medical device to inject and photopolymerize the hydrogel insitu, samples were implanted through an incision of 1 mm into an intervertebral disc of a bovine organ model to evaluate their long-term performance. When implanted into the bovine disc model, the composite hydrogel implant was able to significantly re-establish disc height after surgery (p < 0.0025). The height was maintained after 0.5 million loading cycles (p < 0.025). The mechanical resistance of the novel composite hydrogel material combined with the minimally invasive implantation procedure into a bovine disc resulted in a promising functional orthopedic implant for the replacement of the nucleus pulposus.
