Resumo Objetivos identificar a prevalência de adesão e não adesão à consulta puerperal, assim como facilitadores e barreiras, entre puérperas assistidas em um hospital de ensino. Método estudo de coorte prospectivo, realizado com 121 puérperas, no período de agosto a dezembro de 2019, nas dependências de um hospital de ensino do interior de Minas Gerais. Resultados a prevalência de adesão à consulta puerperal foi de 34,7%. Observou-se, como facilitador, o acolhimento da equipe durante o pré-natal e/ou parto. Citaram-se como barreiras: esquecimento; intercorrências com o RN e/ou puerperais; dificuldade de transporte e distância entre o serviço e a residência. Foram associados à adesão: puérperas com maior escolaridade, que realizaram todo ou parte do pré-natal na instituição, que tiveram a gestação classificada como alto risco, que apresentaram doenças prévias durante a gestação, primigestas e as que tiveram parto cesáreo. Conclusões e implicações para a prática os dados apresentados possibilitaram delinear um perfil das puérperas que aderiram ou não à consulta puerperal, desvelando fatores facilitadores e barreiras, assim como fatores associados à maior adesão. Faz-se necessário repensar a assistência ao puerpério, uma vez que a consulta é uma estratégia de prevenção de morte materna.

The aim of the present study was to identify how the transition of care from the hospital to the community occurs from the perspective of puerperal women at risk. An integrative literature review was performed, with the question: “How does the transition of care for at-risk puerperal women from the hospital to the community occur?” The search period ranged from 2013 to 2020, in the following databases: PubMed, LILACS, SciELO, and Scopus. MESH, DeCS and Boolean operators “OR” and “AND” are used in the following crossover analysis: patient transfer OR transition care OR continuity of patient care OR patient discharge AND postpartum period, resulting in 6 articles. The findings denote discontinuity of care, given the frequency of non-adherence to the puerperal consultation. Transition studies of care in the puerperium were not found, which requires proposing new studies.

Resumen Objetivo: identificar el perfil de embarazos y la prevalencia de adherencia a las consultas puerperales entre madres adolescentes frente a las no adolescentes, atendidas en un hospital clínico universitario en el interior de Minas Gerais (Brasil). Método: estudio transversal anidado en un grupo de puérperas; muestra no probabilística, por conveniencia; embarazo adolescente - variable dependiente; variables sociodemográficas, clínicas y obstétricas- variables independientes. Se utilizó instrumento propio, prueba piloto. Se calcularon razones de prevalencia e intervalos de confianza; Se aplicaron las pruebas chi-cuadrado y exacta de Fisher, considerando un nivel de significancia del 5%, y regresión de Poisson con varianza robusta. Resultados: se entrevistaron a 121 puérperas, el 18,2% (22) eran adolescentes, siendo confirmado entre ellas una baja escolaridad (p<0,001); menor número de embarazos con patologías (p = 0,016); predominando las primíparas (p<0,001) y mayores tasas de parto normal (p = 0,032). La prevalencia de adherencia a la consulta puerperal fue del 34,7% y de 31,8% en adolescentes. No hubo diferencias en cuanto a la adherencia y la edad de las puérperas. Conclusión: las adolescentes no presentaron resultados obstétricos y neonatales negativos, aunque se observó menor escolaridad. Se identificó una asociación entre la edad precoz y la ausencia de enfermedades durante el embarazo y mayores tasas de partos vaginales normales. La adherencia al retorno puerperal fue ligeramente inferior, pero sin significación estadística.
