Adenoviral‐induced rash and mucositis: Expanding the spectrum of reactive infectious mucocutaneous eruption

Abstract: Mucocutaneous eruptions associated with respiratory pathogens, specifically Mycoplasma pneumoniae (MP), has recently been described as a MIRM (MP-induced rash and mucositis). The term reactive infectious mucocutaneous eruption (RIME) has been proposed, since non-MP pathogens may also cause a similar rash and mucositis. We report two cases with clinical manifestations suggestive of MIRM/RIME, both with documented adenovirus infection.

“…Canavan et al named this dermatosis Mycoplasma pneumoniae -induced rash and mucositis (MIRM) and classified it as distinct from SJS/TEN and EM. Subsequently, multiple other studies implicated other infections as causes of MIRM-like reactions including adenovirus [ 64 ], influenza B [ 65 ], and Chlamydia pneumoniae [ 66 ].…”

“… 2 In recent years, other infectious etiologies such as Chlamydia pneumoniae , influenza B, adenovirus, and group A Streptococcus have been described to produce mucocutaneous eruptions indistinguishable from MIRM. 3 , 4 , 5 , 6 , 7 , 8 This has led to the term reactive infectious mucocutaneous eruption (RIME) as a reflection that other infections may stimulate this mucocutaneous eruption. Recurrent episodes are poorly understood.…”

“…MIRM is an entity distinct from erythema multiforme (EM) and Stevens‐Johnson syndrome (SJS) due to its predominance of mucosal involvement and better prognosis than the latter 2 . In recent years, other infectious etiologies such as Chlamydia pneumoniae , influenza B, adenovirus, and group A Streptococcus have been described to produce mucocutaneous eruptions indistinguishable from MIRM 3‐8 . This has led to the term reactive infectious mucocutaneous eruption (RIME) as a reflection that other infections may stimulate this mucocutaneous eruption.…”

“…RIME unifies mycoplasma-induced rash and mucositis (MIRM) and other clinically similar mucosal-predominant eruptions related to a growing number of viral infections, including COVID-19. 2 , 7 12 RIME presents with a prodrome of respiratory symptoms during its 2- to 3-week incubation period. Nearly one-quarter of MP pneumonia patients develop mucocutaneous disease, while RIME manifests in 7%.…”
