Myasthenia gravis is a neuroimmunological disorder leading to skeletal muscle weakness. Common symptoms of the disease, such as anxiety, depression, and insomnia, can cause significant distress in patients. Unfortunately, selecting an appropriate medication for treatment of psychiatric comorbidities can prove to be challenging for providers given the unique pharmacologic constraints that myasthenia gravis presents. The authors present the following clinical vignette and accompanying discussion in an attempt to highlight the special considerations that must be taken into account when treating anxiety and insomnia in patients with myasthenia gravis, as well as to provide an overview of available medication options through the lens of existing constraints.

The main augmentation strategy in the treatment of obsessive-compulsive disorder (OCD) is the addition of low-dose dopamine antagonists, such as risperidone. However, the development of additional pharmacological therapeutics is necessary because some patients remain refractory to these strategies. In the present report, we describe an adult male patient with clomipramine treatment-resistant OCD who did not respond to augmentation with risperidone and aripiprazole but who showed clinical improvement with agomelatine. The effect of agomelatine in resynchronizing circadian rhythms may demonstrate the importance of the circadian rhythm disruption observed in OCD. Moreover, the regulation of the serotoninergic system is circadian in nature, and the resynchronization of the serotoninergic system may regulate serotoninergic dysfunction, a major factor in OCD.

Tratando o transtorno obsessivo-compulsivo refratário: o que fazer quando tratamentos convencionais falham? Treating refractory obsessive-compulsive disorder:what to do when conventional treatment fails? AbstractObsessive-compulsive disorder (OCD) is a chronic and impairing condition. A very small percentage of patients become asymptomatic after treatment. The purpose of this paper was to review the alternative therapies available for OCD when conventional treatment fails. Data were extracted from controlled clinical studies (evidence-based medicine) published on the MEDLINE and Science Citation Index/Web of Science databases between 1975 and 2012. Findings are discussed and suggest that clinicians dealing with refractory OCD patients should: 1) review intrinsic phenomenological aspects of OCD, which could lead to different interpretations and treatment choices; 2) review extrinsic phenomenological aspects of OCD, especially family accommodation, which may be a risk factor for non-response; 3) consider non-conventional pharmacological approaches; 4) consider non--conventional psychotherapeutic approaches; and 5) consider neurobiological approaches.
