Acute renal failure in a patient with mantle cell lymphoma

Abstract: Mantle cell lymphoma is a rare form of B-cell lymphoma. We present a 54-year-old gentleman with mantle cell lymphoma. It was diagnosed based on the demonstration of B-cell antigens CD20 and CD5. It was further confirmed by demonstration of overexpression of cyclin D1 on these atypical lymphocytes in the immunohistochemical staining. He also had acute renal failure and proteinuria. Renal biopsy revealed crescents and lymphomatous infiltration of tubulointerstitium. The presence of infiltrating cells with simila… Show more

“…To date, there are 21 reported cases of MCL with renal involvement; 10 cases with renal MCL infiltration, 3 cases with proliferative glomerulonephritis, 4 cases with membranoproliferative glomerulonephritis, 2 cases with minimal change disease, 2 cases with focal segmental glomerulosclerosis, 1 case with immune complex-mediated glomerulonephritis, and 5 cases with ANCA-negative crescent formation (Table 1). Out of 5 cases with crescents, 3 cases had concomitant lymphomatous infiltration of tubulointerstitium as seen in our case [15, 20, 21]. Out of the 21 cases reported, 15 cases mention the result of ANCA and all were reported negative.…”

“…Till date, only 4 of these cases have been reported globally [ Table 1 ]. [ 17 18 19 20 ] Interestingly, all of them have presented with crescentic GN and LIN; of these four cases, two were having crescentic MPGN. [ 19 20 ] Our patient was also diagnosed to have crescentic MPGN and LIN.…”

“…The most common sites of extranodal involvement are gastrointestinal, including stomach, colon, liver, and Waldeyer's ring . Genitourinary tract involvement by MCL is rare, with the kidney being the most commonly reported site . Fewer reports describe prostate involvement, and only rarely has urinary bladder involvement been described …”
