Acute Liver Failure

Abstract: Acute liver failure (ALF) is a condition that can rapidly progress to multiorgan failure. This article focuses on the diagnosis and management of ALF. We provide a detailed review of the common etiologies of ALF, including acetaminophen overdose, viral hepatitis, drug-induced liver injury, Wilson's disease, and autoimmune hepatitis. The article then addresses how to recognize ALF and reviews the role of common laboratory and imaging tests in establishing this diagnosis. The remainder of the article details the… Show more

“…In terms of etiology of ALF, acetaminophen (APAP) toxicity is the most prevalent cause in developed countries, whereas viral hepatitis accounts for the majority of ALF cases in developing countries . Other causes of ALF include non‐APAP drug‐induced liver injury (DILI), ischemic hepatitis, autoimmune hepatitis (AIH), fulminant Wilson disease (WD), pregnancy‐associated injury, and indeterminant etiology . Table summarizes the prevalence and evaluation of different etiologies of ALF.…”

“…Hepatitis E virus (HEV) is endemic in parts of northern Africa and southern Asia and rarely seen in Western countries. It is transmitted enterically via contaminated water and food . Young adults and pregnant women (especially in the third trimester) are particularly vulnerable to HEV‐induced ALF .…”

“…Acetaminophen is one of the most widely applied analgesic-antipyretic over-the-counter agents. It is relatively safe at therapeutic doses, but can cause liver injury, like acute centrilobular necrosis 23,24 even acute liver failure 25 at large dose or in certain risk groups. 26 The analysed result implied acetaminophen might cause toxicity even at a therapeutic dose when taken together with psoralen/isopsoralen.…”
