Acute Kidney Injury Following Dermatomyositis

Abstract: Renal involvement in patients with inflammatory myopathies, like dermatomyositis, is rare. We present the case of a woman, aged 66 years, who arrived at the hospital with acute renal failure. She had a diffuse erythematous rash, severe muscle weakness and decreased motor capacity. Inflammatory features of the skin/muscle biopsy along with the dramatic improvement in symptomatology upon the intake of steroids pointed towards dermatomyositis. Recognition of this pattern of events, adopting a multidisciplinary ap… Show more

“…The mechanism involved in JDM combined with renal injury may be multifactorial. Myoglobinuric rhabdomyolysis can cause acute tubular necrosis leading to acute kidney injury ( 12 ). In adults, acute kidney injury could occur in 16/150 inflammatory myopathy patients ( 13 ).…”

“…It was previously considered that rhabdomyolysis leading to acute renal failure was uncommon in PM and DM because the rate and degree of myolysis could be lower than those of myotoxic drug-related necrotizing myopathy [11]. However, myoglobinuria-induced acute tubular necrosis caused by PM or DM has been reported in the literature, especially in patients with DM [12][13][14][15][16][17][18][19][20]. e other type of renal damage related to PM or DM is chronic glomerulonephritis, which is presumed to be immune-mediated, and exhibits a variety of histologic features.…”

“…Surveying the published data revealed a few cases reporting renal involvement in patients with DM and immunoglobulin A (IgA) nephropathy 4,6 . However, immunoglobulin M (IgM) nephropathy is yet to be reported.…”

“…Surveying the published data revealed a few cases reporting renal involvement in patients with DM and immunoglobulin A (IgA) nephropathy. 4,6 However, immunoglobulin M (IgM) nephropathy is yet to be reported. Here, we report the case of a 38-year-old man diagnosed with DM following coronavirus disease 2019 (COVID- 19) vaccination, complicated by IgM nephropathy.…”
