Abstract-Aortic wall complications can occur in unrepaired aortic coarctation (CoA) and after surgical repair or endovascular treatment. This review summarizes the available literature and current understanding of aortic wall injury (AWI) surrounding the management of CoA, focusing specifically on acute and follow-up AWI after endovascular treatment.There have been 23 reported cases of aortic rupture after endovascular treatment for CoA, including angioplasty alone, bare metal stenting, and primary covered stent therapy. Even if these published cases represent only a minority of ruptures that have actually occurred, the incidence is substantially <1%. The incidence of acute aneurysm formation was 0% to 13% after angioplasty, 0% to 5% after bare metal stent placement, and <1% after covered stent placement. The reported incidence and natural history of both acute and new AWI during follow-up after endovascular therapy for CoA varies considerably, likely secondary to ascertainment and reporting biases and inconsistent definitions. Although important AWI after endovascular treatment of CoA seems to be declining in frequency with increasing experience and improving technology, it remains one of the most important potential adverse outcomes. Long-term surveillance for new AWI and monitoring of existing AWI is mandatory, with institution of appropriate treatment when necessary. A central research focus in this population should be determination of the appropriate treatment for both native and recurrent CoA across various ages with regard to limiting recurrent CoA and preventing associated aortic wall complications, in addition to determining the appropriate treatment of various AWI. Consistent definitions and reporting are necessary to truly understand the incidence of, risk factors for, and measures protective against AWI after angioplasty or stent implantation for CoA. Before reviewing AWI related to balloon angioplasty (BA) and stent therapy, it is important to set the context by providing an overview of historical, anatomic, and physiological factors that may be related to aortic wall complications in CoA patients, as well as a brief discussion of AWI in patients with surgically repaired CoA.

WstępKoarktacja aorty (CoA) jest czwartą co do częstości występowania wadą układu sercowo-naczyniowego. Wystę-puje u 7% pacjentów z wrodzonymi wadami serca, częściej (ok. 2 razy) u osób płci męskiej. Jest to zazwyczaj zwęże-nie cieśni aorty, tj. odcinka tętnicy głównej między odejś -ciem lewej tętnicy podobojczykowej a przyczepem więza-dła tętniczego (pozostałością drożnego przetrwałego przewodu tętniczego -PDA). Historia naturalna wg opisów z początku XX wieku wskazywała na średni czas przeżycia osób z tą wadą wynoszący 31 lat, a główną przyczyną zgonu były komplikacje -niewydolność serca u 26% chorych, pęknięcie aorty u 21%, bakteryjne endarteritis u 18% i krwawienie śródczaszkowe u 12%. Morfologia i fizjopatologiaKoarktacja aorty może mieć charakter zlokalizowany (typu klepsydry oraz typu błoniastego) lub rozlany (dłuższy odcinek hipoplazji naczynia). Obie te postacie mogą wystę-pować oddzielnie, jakkolwiek dość często współistnieją ze sobą (hipoplazja z towarzyszącym zlokalizowanym zwę-żeniem). Koarktacji aorty często towarzyszą inne wady serca -ubytek w przegrodzie międzykomorowej, PDA i dwupłatkowa zastawka aorty (u blisko połowy chorych). W zespole Turnera CoA stwierdza się u ok. 10% chorych. W zależności od miejsca zwężenia rozróżnia się CoA przedprzewodową, okołoprzewodową oraz pozaprzewodową. W CoA przedprzewodowej, typu noworodkowego, dolna połowa ciała zaopatrywana jest przez PDA (w tym przypadku za mechanizm powstania zwężenia uważa się napełzanie kurczliwej tkanki mięśniowej z PDA na okolicę cieśni). Przeciek przez przewód tętniczy ma kierunek prawo-lewy, tzn. od tętnicy płucnej do tętnicy głównej. Możli-we jest to dzięki wysokiemu oporowi naczyń płucnych w krążeniu małym, a co za tym idzie -podwyższonemu ciśnieniu w tętnicy płucnej. W CoA pozaprzewodowej, typu dorosłego, mechanizmem odpowiedzialnym za powstanie zwężenia wydaje się zmniejszony przepływ przez aortę w tym miejscu. Objawy kliniczne i rozpoznanieNiezależnie od typu CoA w konsekwencji wady powstaje nadciśnienie w częściach ciała zaopatrywanych przez tętnice odchodzące od łuku aorty (kończynach górnych oraz głowie) oraz obniżone ciśnienie na kończynach dolnych. Klinicznie manifestuje się to asymetrią tętna między kończynami górnymi (twarde i wysokie) a dolnymi (słabe lub niewyczuwalne). Zwykle jest również słyszalny cichy późnoskurczowy szmer w lewej okolicy międzyłopatkowej. Odzwierciedleniem stopnia zwężenia jest różnica ciśnie-nia tętniczego między kończynami górnymi i dolnymi. W CoA stwierdza się wyższe ciśnienie na kończynach gór-nych, podczas gdy fizjologicznie powinno ono być nieco
