Background and Purpose. The effect of acupuncture as treatment for poststroke complications is questionable. We performed a randomized, sham-controlled double-blind study to investigate it. Methods. Patients with first-time acute stroke were randomized to receive 24 sessions of either real or sham acupuncture during an eight-week period. The primary outcome measure was change in National Institute of Health Stroke Scale (NIHSS) score. Secondary outcome measures included changes in Barthel Index (BI), Instrumental Activities of Daily Living (IADL), Hamilton Depression Rating Scale (HAM-D), and Visual Analogue Scale (VAS) for pain scores. Results. Of the 52 patients who were randomized to receive acupuncture (n = 28) or placebo (n = 24), 10 patients in the acupuncture group and 9 patients in the placebo group failed to complete the treatment. In total, 18 patients in the acupuncture group and 15 patients in the control group completed the treatment course. Reduction in pain was significantly greater in the acupuncture group than in the control group (p value = 0.04). There were no significant differences in the other measures between the two groups. Conclusions. Acupuncture provided more effective poststroke pain relief than sham acupuncture treatment. However, acupuncture had no better effect on neurological, functional, and psychological improvement.

Сеченова ПОСТИНСУЛЬТНЫЕ ДВИГАТЕЛЬНЫЕ НАРУШЕНИЯДвигательные нарушения в виде центрального гемипареза представляют наиболее частую причину инвалидности паци-ентов, перенесших инсульт. Активизация пациента составляет основу ведения пациентов с постинсультным гемипарезом, лечебная гимнастика направлена на тренировку движений в паретичных конечностях и предотвращение контрактур. У пациентов, у которых высокий мышечный тонус (спастичность) ухудшает двигательные функции, могут быть эффективны пероральные антиспастические средства и(или) местное ведение препаратов ботулинического токсина. Представлены результаты недавно опубликованного исследования CARS, в котором у 208 пациентов (средний возраст 64 года) изучалась эффективность церебролизина (30 мл внутривенно в течение 21 дня) в сравнении с плацебо; показано достоверное улуч-шение восстановления функции паретичной руки у пациентов, получавших церебролизин, в сравнении с группой плацебо. Внедрение в клиническую практику современных методов лечения последствий инсульта позволяет улучшить восстанов-ление и качество жизни больных. Activation of a patient makes the foundation of management of patients with post-stroke hemiparesis, the remedial gymnastics is aimed at training of movements in paretic extremities and prevention of contractions. In patients in whom high muscular tonus (spasticity) impairs motor functions, peroral anti-spastic drugs and (or) local injection of botulinic toxin can be effective. Results of the recently published study CARS are provided in which effectiveness of cerebrolysin (30 ml of intravenous injections for 21 day) was studied on 208 patients (mean age is 64 years) as compared to placebo; statistically significant improvement of the paretic hand function restoration was shown in patients who received cerebrolysin as compared to the placebo group. Implementation in the clinical practice of modern therapeutic methods of stroke consequences allows improving restoration and quality of life of patients.

Synopsis Given the severity of their illness and lack of effective disease modifying agents, it is not surprising that most patients with ALS consider trying complementary and alternative therapies. Some of the most commonly considered alternative therapies include special diets, nutritional supplements, cannabis, acupuncture, chelation and energy healing. This chapter reviews these in detail. We also describe 3 models by which physicians may frame discussions about alternative therapies: paternalism, autonomy and shared decision making. Finally, we review a program called ALSUntangled which using shared shared decision making to review alternative therapies for ALS.
