Actinomycosis of tongue: Rare presentation mimicking malignancy with literature review and imaging features

Abstract: Actinomycosis of the tongue is rare. It may be difficult to differentiate this infection clinically and radiologically from other tongue pathology especially neoplasia. We report a substantial tongue lesion which mimicked malignancy at presentation. The patient was treated successfully with 4 weeks of oral antibiotic therapy. It is also important that clinicians are aware of the radiological differential diagnosis of lingual actinomycosis.

“…Nevertheless, in our case, the TPHA (Treponema Pallidum Hemoagglutination Assay) was negative at admission in the hospital [ 60 , 61 ]. Tuberculosis causes ulcerations as well, affecting mainly the tongue and the palate, and usually have an “infiltrating” feature more than exophytic [ 62 ]; furthermore, pathology can reveal typical caseous tuberculous granulomas which can drive the diagnosis.…”

“…Both children had polymicrobial infection, which is commonly present in actinomycosis and can make diagnosis more challanging. 11 Actinomycosis is rare in childhood but more common in middle aged individuals. It can mimic a wide variety of diseases, including malignancy and tuberculosis and presents with an array of symptoms making diagnosis challenging.…”
