Actinic reticuloid. A clinical, pathologic, and action spectrum study

Abstract: A 54-year-old man suffered from a chronic dermatitis that was more severe on light-exposed areas. Skin biopsy specimens revealed histopathologic findings consistent with those seen in mycosis fungoides. Ultrastructural studies demonstrated the existence of lymphoid cells with hyperconvoluted nuclei, typical of the mycosis fungoides cell or Sézary cell, within the epidermis, dermis, and peripheral blood. Light testing with a monochromator showed abnormal photosensitivity to both short- and long-wave ultraviolet… Show more

“…In addition, the majority had abnormal qualitative responses to UV light and several also reacted to visible light. This is in accordance with other reports on PLR (5,7,10,17,18), although sometimes the threshold to UVB can be within normal limits. A few studies have failed to produce abnormal reactions to UV A testing; a possible explanation is that the given doses of UV A were sub- erythemic.…”

“…Similar patients have recently been reported by Johnson, Cripps & Norback (1979) and Zugerman, Beeaff & Roenigk (1980). The former authors found 10% Sezary cells by electron microscopy ofthe buflfy coat, the latter only a few.…”

“…Occasional Sezary-like cells were also seen in skin and lymph nodes. Johnson et al (1979) speculated that Sezary cell production in actinic reticuloid might be due to chronic exposure of lymphocytes to a photo-allergen.…”
