Objetivo: Avaliar o uso do USG à beira do leito, como guia para o posicionamento rápido e correto do eletrodo do marcapasso, em comparação ao método às cegas. Métodos: Estudo observacional retrospectivo realizado em um único centro, com dois grupos, o primeiro guiado por ultrassonografia e o outro cego. A escolha da técnica utilizada ficou a critério do profissional executor. A amostra foi composta por 60 pacientes internados no período do agosto a outubro de 2020, sendo 28 guiados por ultrassonografia e 32 cegos. Resultados: No grupo cego, a taquicardia ventricular foi a mais frequente (6,3%), em um grupo houve 1 caso de perfuração cardíaca e óbito. No tempo para impactação observou-se diferença estatisticamente significante (*p = 0,0044) entre os grupos avaliados. No grupo com ultrassonografia, a maior proporção de pacientes obteve tempo de impactação inferior a 1 minuto (35,7%). Conclusão: A estratégia do uso da ultrassonografia diminui a taxa de taquicardia ventricular, além de reduzir substancialmente o tempo de impactação para menos de 1 minuto e zerar a necessidade de transferência para hemodinâmica.
