A systematic review and meta‐analysis of outcomes after elective surgery for ulcerative colitis

Abstract: Aim Approximately 20%–30% of patients with ulcerative colitis (UC) will undergo surgery during their disease course, the vast majority being elective due to chronic refractory disease. The risks of elective surgery are reported variably. The aim of this systematic review and meta‐analysis is to summarize the outcomes after elective surgery for UC. Methods A systematic review was conducted that analysed studies reporting outcomes for elective surgery in the modern era (>2002). It was prospectively registered on… Show more

“…Because IBD are accompanied by the synthesis and release of inflammatory cytokines, aminosalicylates and anti-inflammatory drugs, such as steroids, glucocorticoids and immune suppressors, remain the most effective IBD therapies [ 12 , 13 ]. Despite significant progress in understanding the molecular mechanisms behind IBD and improvement in therapeutic strategies, about 20–30% of patients with UC still need surgery [ 14 , 15 ]. Therefore, the search and implementation of new treatments for IBD remains highly relevant.…”

“…11,12 Even if curative for ulcerative colitis, colectomy is associated with short-and long-term complications of the ileal pouchitis and negative consequences such as infertility. 13,14 For these reasons, multiple rounds of advanced pharmaceutical treatment are often attempted before consideration of colectomy.…”

“…Initial therapeutic approach is based in different classes of medicine: antiinflammatory, immunosuppressant (aminosalicylates, corticosteroids, thiopurines) and biological treatment as anti-tumor necrosis factor (anti-TNF), anti-integrins, anti-jak and other. However, most patients have successful clinical control and good evolution approximately 20-30% of patients will require surgery during their life [48].…”
