Tinnitus is believed to result from the maladaptive plasticity of the auditory nervous system; reports regarding its severity and prognosis are conflicting. We evaluated the characteristic and short-term prognosis of tinnitus associated with sudden sensorineural hearing loss (SSNHL). A total of 230 cases were enrolled. The severity and 1-month prognosis of tinnitus (according to the Tinnitus Handicap Inventory (THI)) were assessed in terms of the patients' sex, age, level of hearing loss, type of audiogram results, and so on. According to our statistical analysis, the degree of handicap due to tinnitus was not related to sex, age, or level of hearing loss; the Tinnitus Handicap Inventory indicated that the low-frequency-audiogram group had a low tinnitus handicap (F = 7.516, P = 0.000). Furthermore, we found that the prognosis of tinnitus was not related to the type of audiogram or level of hearing loss. Recovery from a severe level of hearing loss was, however, found to be associated with a poor tinnitus prognosis (F = 5.203, P = 0.006). In summary, our study indicates that the association between tinnitus and SSNHL is extremely high. Tinnitus can be ameliorated by the successful treatment of hearing loss. The study was registered in the Chinese Clinical Trial Registry (ChiCTR1800014797).

Background: To assess the outcomes of intratympanic steroids in the management of idiopathic sudden sensori-neural hearing loss. Study Design: Retrospective study. Setting: tertiary care medical centre. Methods: A total of 56patients with idiopathic sudden sensorineural hearing loss were included in our study. They received 4 doses of 0.3-0.5ml of(40 mg/mL) of methylprednisolone injected into the middle ear with a gap of four days in between the doses. Pre-and postinjection hearing evaluation was done to determine overall success, morbidity, and prognostic factors. Patient variables as they related to recovery were studied and included patient's age, time to onset of therapy, severity of hearing loss, and presence of associated symptoms. Results: A total of 56 patients were included in our study of which unilateral cases were 52 and bilateral cases were 4. Subjectively 38 patients reported improvement in hearing after the therapy while objectively we found 44 ears(74%) ,41 patients had improvement. The mean PTA pre ITS (intratympanic steroid) was 58.7 dB and after ITS it was 30.3dB. Thus mean PTA improved by 28.4 dB. Conclusion: Intratympanic steroids can be offered as a first line therapy for idiopathic sudden sensorineural hearing loss as it is minimally invasive, and can be performed as an office based procedure with no systemic side effects. No major complications has been reported in our study and results have been satisfactory.

Sudden sensorineural hearing loss seems to become a serious social health problem in modern societies. According to the World Health Organization (WHO) reports, adult-onset sensorineural hearing loss is found to be one of the leading diseases at the global level, especially in high-income countries, and is foreseen to move up from the 14th to 7th leading cause of the global burden of diseases by the year 2030. Although the direct mortality rate of this disease is very low, its influence on quality of life is huge; that is the reason why the implementation of the most effective and the safest therapies for the patient is crucial for minimizing the risk of complications and adverse reactions to treatment. The aim of this paper is to present hyperbaric oxygen therapy (HBOT) as a medical procedure useful in the treatment of sudden sensorineural hearing loss as adjunctive therapy of high efficacy. This paper focuses on the molecular mechanisms of action and clinical effectiveness of HBOT in the treatment of idiopathic sudden deafness, taking into consideration both the benefits and potential risks of its implementation.
