A rare case of intussusception through a prolapsed end colostomy

Abstract: Intussusception through a prolapsed end colostomy is amongst the rarest of stoma complications. In this case report, we discuss the therapeutic approach to this complication and provide an update of the existing literature. A 66-year-old male presented with a prolapsed colostomy that was ischaemic in appearance. Intraoperatively, a small bowel intussusception within the prolapsed colon was identified. A subtotal colectomy was subsequently performed to prevent further volvulus formation. When patients present w… Show more

“…An intussusception through a prolapsed colostomy should be differentiated from just prolapsed stoma. Although clinical characteristics that may raise suspension of intussusception include significant irreducible prolapsed segment along with reduced stoma output, patients may present with non-specific symptoms like abdominal distention, vomiting suggestive of intestinal obstruction, or ischemic changes of prolapsed bowel [7] , [8] , [9] , [10] , [11] .…”

“…The application of a CT scan in case of intussusception through a prolapsed colostomy can be diagnostic, showing the affected segment [8] , [10] . On the other hand, it can be non-conclusive based on Temperley et al and Dan et al Reports, necessitating surgical exploration [9] , [11] . For our case patient was taken for urgent surgical exploration due to her acute presentation of a one-day history of long segment prolapse (15 cm).…”

“…We conducted a literature search using PubMed for the terms "stoma prolapse," "hernia," "small bowel incarceration," and "small bowel strangulation" to identify similar cases in the literature. It is very rare for an incarcerated small intestine to enter into a prolapsed end colonic stoma, and only four such cases have been reported previously [13][14][15][16]. The previous 4 cases rarely mention parastomal hernia, but according to the reported clinical findings, they can be considered intrastomal hernia.…”
