A Prospective Study to Evaluate the Role of Complexed Prostate Specific Antigen and Free/Total Prostate Specific Antigen Ratio for the Diagnosis of Prostate Cancer

Abstract: Our study revealed that the overall diagnostic performance of Bayer complexed PSA appears to be better than the other PSA tests and ratios studied. The use of Bayer complexed PSA may lead to a reduction in the number of men undergoing unnecessary prostatic biopsy.

“…However, nonmalignant prostatic diseases, especially benign prostatic hyperplasia (BPH) and acute prostatitis, also cause serum PSA elevation, thus complicating the diagnosis of prostatic cancer by PSA measurements alone (Polascik et al, 1999). The evaluation of the molecular forms of PSA improves the specificity of PSA (Mitchell et al, 2001;Stephan et al, 2002). Despite the availability of these tests, there is an urgent need for new biomarkers for early detection of prostate cancer.…”

“…[18][19][20][21][22] When PCa patients compared with healthy people, sensitivity and specificity values of serum PSA levels are about 75% and 55%, respectively. Many of the patients subjected to a biopsy procedure after a screening test received BPH rather than a PCa diagnosis and people were faced complications and costs of unnecessary biopsies.…”

“…In studies with an overrepresentation of patients with BPH at lower PSA levels and patients with prostate cancer at higher PSA levels, cPSA enhanced the specificity of tPSA to levels equal or even better than that of %fPSA. 16,17 However, other studies using more equal tPSA distributions in patients with BPH and prostate cancer could only show a slightly better diagnostic performance of cPSA alone compared with tPSA. 36,38,39 Only the cPSA/tPSA ratio could reach specificity levels comparable to %fPSA.…”

“…The Bayer cPSA assay measures both PSA-ACT and PSA-API and has been proposed as a single assay, as an alternative to the fPSA and tPSA assays. 16,17 Other existing PSA complexes are so far not measurable in serum. 18 fPSA has recently been shown to exist in three molecular forms; proPSA, 19 BPSA, 20 a special clipped form of fPSA, and inactive "intact" PSA (fPSA-I).…”
