Background: Nonadherence to medication is a recognized problem and may be the most challenging aspect of treatment. Methods:We performed a systematic review of factors that influence adherence and the consequences of nonadherence to the patient, healthcare system and society, in patients with schizophrenia. Particular attention was given to the effect of nonadherence on hospitalization rates, as a key driver of increased costs of care. A qualitative systematic literature review was conducted using a broad search strategy using disease and adherence terms. Due to the large number of abstracts identified, article selection was based on studies with larger sample sizes published after 2001. Thirty-seven full papers were included: 15 studies on drivers and 22 on consequences, of which 12 assessed the link between nonadherence and hospitalization. Results: Key drivers of nonadherence included lack of insight, medication beliefs and substance abuse. Key consequences of nonadherence included greater risk of relapse, hospitalization and suicide. Factors positively related to adherence were a good therapeutic relationship with physician and perception of benefits of medication. The most frequently reported driver and consequence were lack of insight and greater risk of hospitalization respectively. Conclusions: Improving adherence in schizophrenia may have a considerable positive impact on patients and society. This can be achieved by focusing on the identified multitude of factors driving nonadherence.

La adhesión al tratamiento antirretroviral es un factor a considerar en el control del VIH/SIDA. El presente estudio longitudinal tiene como objetivo la exploración de la relación existente entre la adhesión al régimen de tratamiento VIH/SIDA y las variables autoeficacia, esfuerzo y expectativas de control en una muestra de población drogodependiente en programa de metadona. A través de un cuestionario y con un intervalo anual se estudió la evolución de 100 pacientes VIH+. Los participantes se clasificaron en cuatro grupos: adherentes, no adherentes, los que abandonan o inician el tratamiento, según intervalos temporales en los que se recogió la información (día, semana, mes anterior). Se analizaron las relaciones que presentaban con las variables estudiadas encontrándose diferencias en las mismas entre los cuatro grupos. La autoeficacia, el esfuerzo y las expectativas de control se asociaron al inicio, al mantenimiento, a la no adhesión y al abandono del régimen de tratamiento. Abstract The adherence to treatment regimens to Highly Active Antiretroviral Therapies (HAART) is the problem for the success of the viral infection. In this longitudinal study was to explore the relationship of IDUs between adherence to HAART by self-efficacy, effort and control expectancy a sample of 100 HIV+ patients on a methadone maintenance program in Madrid (Spain). They were elicited using a structured, interviewer-administered questionnaire and they were followed for one year. We have split up the sample with the rigourous adhesion criteria (>85% pill intake) and we were been a analysed the relation the adherence to HAART over time with these variables. We describe the following 4 research team: the patients was continued, was refused, was started and the patients was the treatment failure. The results considering that detected significant relations between these variables and the adherence to HAART in the start, continuation, refuse and to medical treatment failure.

Despite the small sample size, our findings are highly relevant as they suggest that, as in AD, 50 mg/day amisulpride is associated with >40% occupancy and clinically relevant responses in VLOSLP. It was not possible to fully characterise concentration-occupancy relationships in VLOSLP, and it is thus unclear whether the greater susceptibility of those with AD to emergent EPS was accounted for by increased central drug access. Further investigation of age- and diagnosis-specific threshold sensitivities is warranted, to guide amisulpride prescribing in older people, and therapeutic drug monitoring studies offer a potentially informative future approach.
