A new positive SARS-CoV-2 test months after severe COVID-19 illness: reinfection or intermittent viral shedding?

Abstract: We present a case of a patient who had a history of severe coronavirus disease (COVID-19) 4 months prior to this current presentation and, after a long asymptomatic period, subsequently tested positive for severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) by a RNA PCR assay, after several interval negative SARS-CoV-2 RNA tests. We present this potential case of SARS-CoV-2 reinfection in order to incite discussion around differentiating persistent infection with intermittent viral shedding and reinfe… Show more

“…Viral cultures can also provide information in this regard – if viral cultures are negative, then viral shedding is more likely while identifying viable viruses argues for reinfection or reactivation. Clinical features may also aid in the differential diagnosis: the elapsed time from the first RT-PCR confirmation – longer time intervals being associated with a higher probability of reinfection; the presence of symptoms is also likely to suggest reinfection most patients with prolonged viral shedding are asymptomatic ( European Centre for Disease Prevention and Control [ECDC], 2020 ; Tuan et al, 2021 ).…”

“…has not yet been resolved, and COVID-19 will last for a long time. 16 Innate immunity is an important concept in epidemic theory of the population-level effect of individual immunity to prevent the transmission of pathogens. Innate immunity exists when a population or a sufficient number of animals in the population have immunity to a drug, which reduces the likelihood of effective contact between the affected and the vulnerable.…”
