A clinical, electrophysiological, and pathological study of neuropathy in rheumatoid arthritis

Abstract: Neuropathy in rheumatoid arthritis (RA) may result secondary to entrapment, vasculitis, and drug toxicity. We aimed to study clinical and electrophysiological neuropathy and pathological changes in sural nerve in patients with RA. One hundred eight patients of RA, fulfilling American College of Rheumatology 1987 criteria (mean age, 45.83 years; M/F 1:3, 80.3% seropositive) were examined clinically and electrophysiologically for evidence of peripheral neuropathy. Sural nerve biopsies were performed in the invol… Show more

“…In chronic inflammatory conditions such as RA, the peripheral nerves undergo prolonged stress with mechanical compression due to swollen joints and immobilization due to a decrease in activities. As a result, ischaemia, hypoxia, endoneurial oedema, inflammation in the neural tissues and fibrin deposits occur and cause axonal degeneration, demyelination and Schwann-cell necrosis followed by remyelination (2,3,26). In addition, mechanical stress has been shown to stimulate the release of substances P and CGRP, which domized, according to their age ranges, into the control group or the experimental group, with similar clinical demographics.…”

“…In the literature, that value varies from 0.5% to 80%. 36 Clinically detectable involvement of the peripheral nervous system in RA is uncommon. It can be asymptomatic at initial stages or have a large variety of symptoms, such as pain, paresthesia, and muscle weakness.…”
