A 40-year-old woman underwent laparoscopic common iliac lymphadenectomy for metastasis from rectal cancer. Two weeks after the surgery, she was found to have massive chylous ascites. After failure of conservative treatment, bilateral inguinal intranodal lymphangiography was performed. No definite extravasation was observed while lipiodol injected through the left inguinal node was ascending. When we punctured the right inguinal lymph nodes and started the injection of lipiodol, extravasation of diluted lipiodol was noted at the level of the first sacrum. Careful observation revealed that the ascending lipiodol became diluted in the cisterna chyli, refluxed through the median paraaortic route, leaked from the excised left common iliac lymph vessel, and flowed into the abdominal cavity. Lipiodol used in lymphangiography did not reduce chylous ascites at all. Twenty-seven days after lymphangiography, laparoscopic lymphatic ligation was performed, and the chylous ascites disappeared completely. CT obtained 40 days after surgical repair revealed disappearance of ascites and enlargement of the thoracic duct, which had not been observed on preoperative lymphangiography. Notably, lymphatic reflux from the cisterna chili can occur without obstruction of the thoracic duct and may result in chylous ascites.

Chyluria is defined by the presence of chyle in urine, and is caused by a communication between the lymphatic and urinary system. Commonly, it is characterised by the excretion of milky white urine, which is present in up to 70% of chyluria cases. The prevalent cause for chyluria in Asia is filariasis with Wuchereria bancrofti. Non-parasitic chyluria is more common in western countries and is usually subsequent to traumatic factors, infections, or tumours. The occurrence of non-parasitic chyluria in the absence of trauma, iatrogenic or other, is exceedingly rare. The lymphatic system is rather challenging to visualize. Herein, we present a case of non-parasitic chyluria and our approach of combining different complementing imaging modalities, such as contrast-enhanced ultrasound (CEUS) retrograde pyelography and magnetic resonance (MR) lymphography.

Mục tiêu đánh giá tính an toàn của kỹ thuật chụp cộng hưởng từ (CHT) ống ngực trên các bệnh nhân có chẩn đoán rò dưỡng chấp vùng cổ sau mổ ung thư tuyến giáp. Đối tượng và phương pháp: nghiên cứu mô tả hồi cứu trên các bệnh nhân đã thực hiện chụp CHT ống ngực bằng tiêm đối quang từ vào hạch bẹn hai bên. Kết quả: trong thời gian 2019-2023, tổng số 38 bệnh nhân đã thực hiện chụp CHT bạch mạch tiêm đối quang từ vào hạch bẹn. Kết quả thời gian chụp trung bình mỗi bệnh nhân hết 22 phút (15-40 phút). Thời gian thuốc đối quang từ lên đến bể dưỡng chấp trung bình 8 phút từ khi bắt đầu tiêm thuốc đối quang vào hạch bẹn. Biến chứng thường gặp nhất là đau tức tại vị trí tiêm thuốc 19 bệnh nhân (50%) ngay tại thời điểm tiêm thuốc. Sưng nề do thoát thuốc đối quang từ ra ngoài hạch ở 2 bệnh nhân (5,3%). Không có biến chứng nặng cần phải can thiệp điều trị. Kết luận: chụp CHT ống ngực tiêm thuốc đối quang từ hạch bẹn là một phương pháp chẩn đoán ít xâm lấn, an toàn và không có biến chứng cần phải điều trị.
