Megacephaly polymicrogyria, polydactyly, hydrocephalus (MPPH) is an extremely rare condition caused by a defect in the AKT3, CCND2, or PIKR2 genes. Although the prevalence of the syndrome is very low, there is a significant clinical and radiological variation in the syndrome. We present a case with MPPH admitted to the hospital due to an increase in seizure frequency. The patient had a history of cerebral palsy, global developmental delay, spasticity, and hypoglycemic episodes. MRI findings revealed ventriculomegaly, polymicrogyria, abnormal encephalon, and pachygyria. The addition of clobazam and alprazolam diminished the seizures' frequency and the patient's spasticity, respectively. To highlight the clinical and radiological variation of the syndrome, we review cases of MPPH with clinical and radiological variants. Pachygyria and cerebral palsy are new associations not previously described before in MPPH. Pachygyria and cerebral palsy could be worsening the seizures and the global delay in this patient. Hypoglycemic episodes are probably related to the AKT3 gene, promoting more glucose consumption. Spasticity is most probably related to an upper motor sign due to the patient's cerebral palsy. This case highlights the clinical and radiological variation of the syndrome. More cases of MPPH need to be described to consolidate the knowledge and have a better understanding of the clinical and radiological variation of the syndrome.

Abstrak: Tindakan pencegahan penyakit gigi dan mulut yang efektif adalah menyikat gigi, tetapi sikat gigi bisa menjadi sarana transmisi mikroorganisme patogen yang menyebabkan terjadinya penyakit sistemik dan penyakit mulut. Pergantian sikat gigi 3 bulan sekali dianjurkan agar tidak terjadi penumpukan bakteri tetapi rata-rata masyarakat Indonesia menggantinya minimal 10 bulan sekali. Tujuan: Mengetahui penggunaan disinfektan kimia dan disinfektan tradisional terhadap penurunan jumlah mikroba pada sikat gigi. Metode: Literature review, artikel diperoleh melalui Google Scholar database (2016-2021) menggunakan PICOS sebagai penentu kriteria inklusi dan kriteria eksklusi. Hasil: Disinfektan kimia yang efektif adalah NaOCl 2%, listerine, chlorhexidine gluconat, dan povidone iodine, sedangkan disinfektan tradisional yang efektif adalah cuka dengan asam asetat glasial 5-8%, cuka 1%, larutan cuka putih 38% yang dicampur dengan larutan garam 3,5%, larutan cuka putih 50%, larutan cuka putih 38%, bawang putih, perasan jeruk nipis, rebusan bunga rosella, mimba, daun jambu biji, daun teh hijau, daun pepaya, baking soda, garam, dan daun teh hitam. Kesimpulan: Disinfektan kimia yang terbukti mampu mengurangi jumlah bakteri pada sikat gigi adalah NaOCl 2% dan disinfektan tradisional yang terbukti mampu mengurangi jumlah mikroba keseluruhan pada sikat gigi adalah cuka dengan asam asetat glasial 5-8%, serta cuka 1% dan larutan cuka putih 38% yang dicampur larutan garam 3,5% terbukti mampu mengurangi jumlah bakteri keseluruhan pada sikat gigi.
