“…The prevalence of abnormal ventilation in our cohort was high, and this was expected given that 82% of participants were current or former smokers, and 30% had concomitant COPD. The relationship of preoperative VDP with smoking history and COPD observed in our study aligns with previous literature, which reports higher defect burden in COPD patients compared with healthy individuals [ 10 ] and highlights abnormal ventilation in former [ 13 , 21 , 22 ] and current smokers without lung disease [ 23 – 25 ]. Consistent with the notion that ventilation imaging modalities are more sensitive to underlying airway and parenchymal pathology, abnormal ventilation was detected even in patients with normal FEV 1 and D L CO , the guideline-recommended metrics for preoperative lung function assessment.…”

“…Although the ϳ21% difference (M2L vs. M2R) in the SI measurement did not match the predicted ϳ70% difference in the gas velocity, the SI measurement can be calibrated by applying a larger flip angle to match the gas velocity in the major airways. These data demonstrate that a noninvasive measurement of local airflow profile to determine regional lung function in the major airways can be achieved using the HP 3 He dynamic MRI technique. Opposing results were observed in Fig.…”

“…To investigate regional pulmonary function, it is critical to determine airway diameter. To measure airway diameter based on the volume-projection 3 He images, the ROI (i.e., the major airways) must be fully enclosed in the plane of the selected slice. Since the major airways diverge and lean over the dorsa, the animal was tilted from the head (ϳ20°) to include most of the major airways in the selected coronal slice (5 mm in thickness).…”

“…A recent multicentre study showed that the estimated ventilated lung volume correlated with both CT and pulmonary function tests, although these correlations were not perfect (and it was postulated that CT and MRI are complementary techniques in this setting) (29). Other studies have shown similar results (26, 30–33). In normal volunteers, more or less homogeneous ventilation distribution is seen, whereas progressive ventilation defects are present with increasingly severe emphysema, while distribution of these defects is different between smoking‐related emphysema and alpha‐1 antitrypsin deficiency (the latter shows a lower lobe predominance as predicted).…”
