Background: Heart failure is the leading cause of death in dialysis patients. Cardiac arrest due to hypotension may also occur during dialysis therapy. If cardiac arrest is elicited, manual chest compressions (MCC) should be started as soon as possible. However, if MCC is applied during the patient is being treated on the dialysis chair, the backrest of the dialysis chair is horizontal to the floor and there is no support between the backrest and the floor, so that will shake and become unstable.Methods: we investigated whether a round chair is effective for supporting the dialysis chair for MCC. Four adult males performed MCC on three dialysis chairs using a manikin. MCC was performed with 2 sets (1 set was 100 times per minute) per person, with or without a round chair. A total of 4,800 times were performed by four executors.Results: When the chair was not used as a stabilizer, the mean value of fluctuation range were 20.8 ± 8.1 mm, 18.7 ± 5.5 mm, and 12.8 ± 1.8 mm, respectively. When the chair was used, the mean value of fluctuation range were 6.1 ± 1.1 mm, 7.5 ± 2.1 mm, and 1.0 ± 0 mm, decreasing by 70%, 59%, and 92%.Conclusion: MCC with the stool under the backrest as a stabilizer was effective for dialysis chairs.

Background Cardiac arrest is still a global public health problem at present. The neurological outcome is the core indicator of the prognosis of cardiac arrest. However, there is no effective means or tools to predict the neurological outcome of patients with coma and survived 24 hours after successful cardiopulmonary resuscitation (CPR). Hypothesis Therefore, we expect to construct a prediction model to predict the neurological outcome for patients with coma and survived 24 hours after successful CPR. Methods A retrospective cohort study was used to construct a prediction model of the neurological function for patients with coma and survived 24 hours after successful CPR. From January 2007 to December 2015, a total of 262 patients met the inclusion and exclusion criteria. Results The predictive model was developed using preselected variables by a systematic review of the literature. Finally, we get five sets of models (three sets of construction models and two sets of internal verification models) which with similar predictive value. The stepwise model, which including seven variables (age, noncardiac etiology, nonshockable rhythm, bystander CPR, total epinephrine dose, APTT, and SOFA score), was the simplest model, so we choose it as our final predictive model. The area under the ROC curve (AUC), specificity, and sensitivity of the stepwise model were respectively 0.82 (0.77, 0.87), 0.72and 0.82. The AUC, specificity, and sensitivity of the bootstrap stepwise (BS stepwise) model were respectively 0.82 (0.77, 0.87), 0.71, and 0.82. Conclusion This new and validated predictive model may provide individualized estimates of neurological function for patients with coma and survived 24 hours after successful CPR using readily obtained clinical risk factors. External validation studies are required further to demonstrate the model's accuracy in diverse patient populations.

Background: Due to the limited human resource and medical materials, the mortality of children under-5 was high in Tanzania. Systematic and effective training programs for pediatric medical staffs are required. Thus, aim of this study was to describe and test a simulation training program of pediatric technology for doctors and nurses in Zanzibar.Methods: A simulation training program of pediatric appropriated technology was designed and performed in this quasi-experimental study, in which pre-post self-reported of pediatric knowledge (scoring 0-100) of participants were calculated to analysis the effectiveness of this simulation training in Apr, 2018 and Oct, 2018. Correctly answering of items≥80% was considered to be a pass. Data were entered and analyzed in SPSS 22.0 software to test the hypothesis. Results: 56 participants were finally included in this study with 27 doctors and 29nurses. Pre and post test scores of pediatric knowledge were at mean 44.32 (SD 9.53) versus mean 82.11 (SD 11.06) respectively, p<0.0001. The most effective training domain was (pediatric life support) at 44(78.57). Passing rate of pediatric knowledge increased dramatically after training with p value less than 0.0001. Although there was a mild difference between doctors and nurses at scoring of pre-test (p=0.048), the training effect between them had no statistical differences (p>0.05).Conclusion: The scenario simulation training course of pediatric appropriated technology could significantly improve pediatric knowledge for doctors and nurses in Zanzibar. It might be further promoted as one effective module of training in health foreign aid.
