Dengue: 30 years of cases in an endemic area

Conforme a literatura, fatores ambientais, demográficos e socioeconômico têm contribuído para a manutenção dos altos índices de casos de dengue no Ceará. Além disso, a introdução de vírus como Chikungunya e Zika em 2014, agravou o cenário endêmico. Sendo assim, diante das condições favoráveis as endemias e a necessidade permanente de novos estudos para compreender o papel dinâmico das infecções por arboviroses no Ceará, o objetivo deste estudo foi avaliar fatores relacionados à maior incidência de arboviroses em cidades cearenses em 2021.Trata-se de um estudo realizado a partir dos dados divulgados pela Secretaria de Saúde do Governo do Estado do Ceará (SESA). Foram selecionados entre os 184 municípios cearenses, as dez cidades com maior e menor incidência de arboviroses transmitidas pelo Aedes aegypti em 2021, gerando dois grupos e, por conseguinte, foram analisadas as variáveis densidade demográfica, pluviosidade, temperatura, índice de desenvolvimento humano (IDH), escolaridade, renda, mortalidade infantil, estabelecimento de saúde, número de médicos, disponibilidade de água potável, saneamento básico e coleta de resíduos domiciliares. Os dados coletados foram analisados no programa PRIZMA. Os resultados revelaram que os dez municípios com maior incidência de casos de arboviroses apresentam concentração populacional, temperaturas média e mínima, IDH, renda, mortalidade infantil, número de estabelecimentos de saúde, número de médicos, taxa de saneamento e de coleta de resíduos sólidos significativamente maiores quando comparado com os dez municípios com menor incidência. Entretanto, não houve diferença entre os grupos quando se avaliou a temperatura máxima e a pluviosidade. Portanto, sugerimos que órgãos e secretarias ligadas ao controle da dengue, assim como a comunidade intensifiquem as suas ações não somente durante a quadra chuvosa, mas atuem também durante todo o ano, visto que outros fatores contribuem para o aumento dos casos de arboviroses nas cidades cearenses.

Background Dengue is the most rapidly spreading viral vector-borne disease in the world. Promising new dengue vaccines have contributed to a growing consensus that effective dengue control will require integrated strategies of vaccination and vector control. In this qualitative study, we explored the perspectives of residents of Fortaleza, Brazil on acceptability of a hypothetical safe and effective dengue vaccine, specific drivers of dengue vaccine acceptance or hesitance, and the expected impact of dengue vaccination on their personal vector control practices. Methods A total of 43 in-depth interviews were conducted from April to June 2022 with Fortaleza residents from a diverse range of educational and professional backgrounds, with and without recent personal experiences of symptomatic dengue infections. Data were analyzed using the principles of inductive grounded theory methodology. Results Our findings indicate that knowledge of dengue transmission, symptoms, and prevention methods was strong across respondents. Respondents described willingness to accept a hypothetical dengue vaccine for themselves and their children, while emphasizing that the vaccine must be demonstrably safe and effective. Respondents expressed diverse perspectives on how receiving a safe and effective dengue vaccine might influence their personal vector control behaviors, relating these behaviors to their perception of risk from other Aedes mosquito-carried infections and beliefs about the role of vector control in maintaining household cleanliness. Conclusions Our study findings provide community-level perspectives on dengue vaccination and its potential impact on personal vector control behavior for policymakers and program managers in Fortaleza to consider as new dengue vaccines become available. With the introduction of any new dengue vaccine, community perspectives and emerging concerns that may drive vaccine hesitancy should be continuously sought out. Improved urban infrastructure and efforts to engage individuals and communities in vector control may be needed to optimize the impact of future dengue vaccinations and prevent rising cases of other arboviruses such as Zika and chikungunya.

Кольцово, Российская Федерацияцель. разработка диагностического набора для выявления маркеров лихорадки денге на всех стадиях заболевания. материалы и методы. в сыворотках крови от пациентов с подозрением на лихорадку денге проводили выявление антигена NS1 и специфических IgM и IgG методом иммуно-хроматографии, методом дот-иммуноанализа, иммуноферментного анализа, с использованием коммерческих тест-систем, а также экспериментальным набором «денге-спектр». результаты и обсуждение. разработан диагностический набор для выявления маркеров лихорадки денге, основанный на механизме одновременного дифференциального выявления белка NS1 возбудителя и антител класса IgM и класса IgG с образованием специфических комплексов между маркерами из исследуемого образца и известными иммунореагентами захвата, в определенном порядке дискретно зафиксированными на плотной подложке. установлено, что эффективное выявление специфических IgG и Igм к вирусу денге может быть осуществлено по схеме, при которой захват IgG производится на суммарном антигене вируса с детекцией с помощью меченных антител против IgG человека, а выявление Igм осуществляется захватом на антителах против Igм человека с детекцией меченным суммарным вирусным антигеном. выявление белка NS1 вируса денге может быть выполнено с использованием подложки с иммобилизованными моноклональными антителами к NS1 и иммунозоля золота, связанного с антителами к NS1. при такой постановке дот-анализа лимит определения рекомбинантного аналога белка NS1 составил 100 нг/мл. сравнительные испытания набора на панели клинических образцов показали хорошее совпадение результатов с данными, полученными с использованием импортных коммерческих тестов. разработанный набор может найти применение для скрининга клинических образцов, как в лабораторных, так и в полевых условиях.Ключевые слова: вирус денге, дот-иммуноанализ, иммуноферментный анализ.Abstract.
