Normalization of height and excess body fat in children with salt-wasting 21-hydroxylase deficiency

Abstract: Objective: To evaluate growth and body composition of patients with the salt wasting form of classical congenital adrenal hyperplasia due to 21-hydroxylase deficiency and to compare them with healthy children. Methods: Twenty-one prepubertal patients (eight boys and 13 girls) between 2.1 and 10.2 years and 67 prepubertal healthy controls (36 boys and 31 girls) between 1.2 and 11.7 years were included. Weight, height, upper-arm circumference, skinfolds, body composition determined by bioimpedance, and bone age w… Show more

“…Body composition of children with CAH is an emerging area of focus, and recent data indicate that body mass index (BMI) and/or fat mass is elevated in children and adults with CAH . In normal growth, BMI increases rapidly in infancy and peaks at around the first year of life.…”

“…al, 2011; Cetinkaya & Kara, 2011; Moreira et al, 2013). Medes-dos-Santos et al (2011) concluded in their cross sectional, retrospective study that height in these children was similar to unaffected children of the same age, yet children with CAH had higher body fat, which tended to be visceral. Similarly, Moreira et al (2013) found that children with CAH presented with a higher prevalence of obesity and metabolic syndrome, but child weight was not correlated with glucocorticoid treatment.…”

“…Of the ten, five articles focused on growth issues, both height and weight. Obesity was found to be a significant problem in children with CAH (Volkl, Simm, Beier, & Dorr, 2006; Mendes-dos-Santos et. al, 2011; Cetinkaya & Kara, 2011; Moreira et al, 2013).…”

“…Therefore, the goal of therapy in SW-21OHD patients is to normalize the androgen levels and total-body sodium depletion to allow normal growth and development and avoid volume overload (16). In our study, the required glucocorticoid dose (adjusted per square meter) was stable during the first two years of life.…”
