Abstract: Objective: The conversion of Hashimoto's thyroiditis (HT) to hyperthyroidism due to thyrotropin receptor antibodies is intriguing and considered rare. The contribution of TSH receptor blocking antibodies (TRAb), which may be stimulators (TSAb) or blockers (TBAb), is suspected. We describe clinical and biological variables in a series of patients switching from Hashimoto's thyroiditis to Grave's disease. Subjects and methods: Retrospective case study of 24 patients with Hashimoto's thyroiditis followed during 4… Show more

“…Although the traditional view considers GD and HT to be two separate diseases, the present view is that these conditions may be opposite ends of the spectrum of one disease. Indeed, a few researchers have reported a sequential phenotypic conversion from GD to HT or vice versa [ 24 – 26 ]. The underlying mechanism is not clear but may be determined by the following factors: TSAb and TBAb activity, responsiveness of the thyroid gland to TSAb or TBAb, and changes in the thyroid gland [ 23 ].…”

“…The serum thyroid autoantibodies such as TgAb and TPOAb are often also positive in patients with GD ( 53 ). Clinically, transitions of patient from GD to HT and vice versa are not uncommon ( 52 , 54 ), and a positive family history for GD often overlaps with that for HT ( 55 ). Provocation or activation of AITD by COVID-19 toward the seemingly opposite direction, to GD or HT, can be understood from these perspectives ( Supplementary Figure 1 ).…”

“…Although the conversion of HT to GD is a rare phenomenon and its mechanism is still unclear, a few cases were reported in the medical literature. Table 1 provides a summary of some cases (2,4,(12)(13)(14)(15)(16)(17).…”
