Renal ischemia/reperfusion (I/R) injury resulting in acute renal failure, is a major clinical problem due to its high mortality rate. Renal I/R increases the reactive oxygen species, secretion of inflammatory cytokines, chemokines and other factors. This suggests that initiating the apoptosis process in the presence of oxidative stress may play a role in life‐threatening conditions, such as ischemia. Ischemia reperfusion‐induced renal damage can result in renal failure and death. Although many treatment procedures have been carried out to reduce or destroy renal I/R damage in experimental models, so far, a routine method of treatment has not yet been found. For this reason, the current study was planned to investigate the possible protective effects of evodiamine on tissue damage caused by ischemia‐reperfusion in kidney tissue in rats and an experimental renal I/R model was used for this purpose. Four groups were formed in the study: the control, sham control, ischemia reperfusion (I/R), and evodiamine (10 mg/kg) + I/R groups. The effects of evodiamine against kidney I/R injury were investigated. TAS (total oxidant status), TOS (total oxidant status), interleukin‐1β (IL‐1β), IL‐6, IL‐10 and tumor necrosis factor‐α levels were determined by enzyme‐linked immunosorbent assay. The oxidative stress index was calculated from TAS and TOS levels. In addition, the renal ischemia reperfusion injury was examined histopathologically. The IL‐10 and TAS levels in the I/R group decreased when compared with the control and Sham groups, while these levels increased in the evodiamine group. Histopathologic examination revealed that caspase 3 and nuclear factor‐κB levels decreased in the evodiamine group compared with the I/R group. The application of evodiamine significantly reduced ischemia reperfusion‐induced kidney damage due to its antioxidant, anti‐inflammatory and antiapoptotic properties.

Objective To investigate the effect of early-life unilateral testicular torsion on adult male fertility. Methods Clinical information was collected for 122 patients who had experienced unilateral testicular torsion at a median age of 15.5 years. The recent pregnancy rate and time to pregnancy experienced by the patients’ female partners were assessed by structured interviews. Data were analyzed by the chi-squared test and Student’s t-test. Results Seventy-two patients with testicular torsion met the criteria for inclusion in our analyses; 49 had undergone orchiectomy, while 23 had undergone surgical repositioning/orchiopexy. The pregnancy rate and median time to pregnancy were 83.67% (41/49) and 1.6 years, respectively, in the orchiectomy group, whereas they were 91.30% (21/23) and 0.75 years, respectively, in the repositioning/orchiopexy group. The recent pregnancy rate was higher in patients with torsion in childhood than in patients with torsion in adolescence; it was lowest in patients with torsion in adulthood. Surgical repositioning/orchiopexy yielded a significantly better recent pregnancy rate among the three groups, based on age at the time of torsion, and a shorter time to pregnancy than orchiectomy in patients with torsion in adolescence. Conclusion Onset of unilateral testicular torsion early in life has a negligible effect on adult male fertility.

Combined drugs in the treatment of acute respiratory viral infections: a review of research results

Acute respiratory infections are of the greatest economic significance among all infectious diseases in the Russian Federation. There are no drugs with a direct antiviral effect for most acute respiratory viral infections, which efficacy would have been proven in the numerous clinical trials and confirmed by the results of meta-analyses today. The use of various combinations of antipyretic, anti-inflammatory and antioxidant drugs is the most common method of symptomatic and pathogenetic therapy of acute respiratory viral infections (ARVI). The purpose of this review is to analyse and systematize the results of preclinical and clinical trials aimed at studying the safety and efficacy of fixed-doses combinations of paracetamol, pheniramine maleate, phenylephrine hydrochloride and ascorbic acid in the ARVI therapy. The search of scientific publications was carried out in the PubMed, ClinicalKey ELSEVIER and Google Scholar databases. The search depth was 10 years. The results of numerous comparative and placebo-controlled trials showed that the use of fixed-dose combinations of paracetamol, pheniramine maleate, phenylephrine hydrochloride and ascorbic acid in the ARVI therapy was pathogenetically justified, safe, and effective in relieving symptoms such as fever, rhinitis, cough, muscle and joint pain, sore throat, and headache. The combination drugs can ease the patient’s condition and help shorten the duration of the illness provided that they are administered in due time. Meanwhile, the pathogenetic effects of combination drugs aimed at localizing the focus of inflammation and minimizing the risk of complications warrant further research.
