Abstract: Acute pulmonary thromboembolism (APTE) is a highly prevalent condition (104-183 cases per 100,000 person-years) and is potentially fatal. Approximately 20% of patients with APTE are hypotensive, being considered at high risk of death. In such patients, immediate lung reperfusion is necessary in order to reduce right ventricular afterload and to restore hemodynamic stability. To reduce pulmonary vascular resistance in APTE and, consequently, to improve right ventricular function, lung reperfusion strategies hav… Show more

“…Venous thromboembolism (VTE) is a highly prevalent and potentially fatal disease. It is the third most common cause of cardiovascular death, following acute coronary artery disease and stroke, and is responsible for more than 3 million deaths per year worldwide [1,2]. Several risk factors have been associated with VTE such as obesity, hormone use and immobility among others, but none as relevant as cancer.…”

“…The use of systemic thrombolytic agents for the treatment of acute pulmonary embolism is a controversial subject, in which evidence and belief eventually clash. While there is no dispute regarding the benefits of the procedure for high-risk patients, 1 this indication is much more debatable for intermediate-high-risk patients. These latter patients are characterized by maintaining adequate levels of tissue perfusion at the expense of right ventricle injury.…”

“…In the current issue of the JBP, there is a good review of indications for reperfusion, as well as reperfusion modalities, in patients with PTE. 12 Conventional thrombolysis remains the most widely used drug therapy for achieving reperfusion. However, treatment options such as low-dose, peripherally administered thrombolytic therapy or in situ thrombolysis to reduce the risk of bleeding have shown good results; nevertheless, further studies are needed in order to define their therapeutic role.…”
