Abstract: Objective:To evaluate the frequency of and factors associated with indeterminate interferon-gamma release assay (IGRA) results in people living with HIV/AIDS (PLWHA). Methods:We tested 81 PLWHA in the central-west region of Brazil, using the tuberculin skin test and an IGRA. Information on sociodemographic and clinical variables was gathered through the use of questionnaires and from medical records. The association of those variables with indeterminate results was analyzed by calculating the adjusted ORs in a… Show more

“…Further support for the role of impaired immune responses as a cause of INDT tests comes from the similarly high rates of INDT IGRA results described in a variety of other immunocompromised groups including HIV patients, IBD patients on steroids, and rheumatoid arthritis patients …”

“…A recent meta‐analysis demonstrated a pooled incidence rate of INDT QFT‐GIT results of 2.1%, which increased to 4.4% in immunosuppressed patients . Similarly, higher rates of INDT results have been demonstrated in the HIV population, which were attributed to low CD4 counts and a lack of T‐cell response . The potential for poorer performance of IGRAs in immunosuppressed individuals is of particular relevance to liver transplantation, as the majority of patients undergoing liver transplantation have multifaceted immune dysfunction as part of their end‐stage liver disease.…”

“…We found that indeterminate results were reported in 8.3% of the cases, a rate similar to that of the general population but lower than those reported among PLHIV. (21)(22)(23) An altered immune response increases the likelihood of indeterminate results. (11,21,22,(24)(25)(26) HIV can lead to indeterminate results even with immune reconstitution due to altered T-cell function, (27)(28)(29) and a low CD4 cell count is often implicated.…”

“…The QFT-GIT assay is an in vitro blood test based on the release of IFN-γ by T cells in response to stimulation with TB-specific antigens. Thus, IGRA testing in HIV-infected individuals with a reduced T cell count is more likely to result in indeterminate results 18 . Glucocorticoids impair a variety of T cell functions; therefore, high doses can induce T cell apoptosis, which leads to the inhibition of Th1-derived cytokines, such as IFN-γ, interleukin (IL)-2, IL-10, and tumor necrosis factor (TNF) 19 .…”
