| Objectives: To assess whether there is a correlation between oxygen uptake (VO 2 ) and heart rate (HR) on-kinetics in the constant-load cycle-ergometer test (CLT) and the BODE index and its isolated variables in patients with chronic obstructive pulmonary disease (COPD). Method: Fourteen male patients between 55 and 78 years of age with moderate to severe COPD were evaluated. Each patient underwent spirometry, the six-minute walk test (6MWT), the cycle-ergometer incremental test (IT) and CLT on alternate days. The exhaled gases were collected, and the VO 2 and HR on-kinetics were analyzed. The BODE index was calculated. Results: It was noted that the VO 2 tau (τ) and mean response time (MRT) were significantly higher than HR τ and MRT. Moderate and strong correlations between τ and MRT of the VO 2 and HR and the BODE index was noted (r=0.75 and r=0.78; r=0.62 and r=0.63, respectively), and there were correlations between the VO 2 τ and MRT and the forced expiratory volume in one second (FEV 1 ) (r=-0.60; r=-0.53) and the distance traveled at 6MWT (DT-6MWT) (r=-0.61; r=-0.44) and DT-6MWT % predicted (r=-0.62; r=-0.46). The HR τ and MRT were correlated with DT-6MWT (r=-0.59; r=-0.58) and DT-6MWT % predicted (r=-0.62; r=-0.62). Conclusion: The slowing of cycle-ergometer VO 2 , and especially of HR on-kinetics, may be key markers of disease severity. Furthermore, airflow obstruction and reduced exercise capacity are associated with the slowing of patients' VO 2 and HR on-kinetics.

Background: Chronic Obstructive Pulmonary Disease (COPD) is a progressive disease that reduces functional capacity. deteriorating the ability to perform activities of daily living (ADL). A close relationship between morbidity and mortality with functional limitation is observed in patients with COPD. Objectives: To determine if there is a relationship between ADL limitation and the BODE index. which is a predictor of mortality. in patients with moderate to severe COPD. Methods: Thirty-nine patients with COPD GOLD 2 to 4 recruited by convenience. were submitted to the following tests: spirometry. body mass index (BMI). the London Chest Activity of Daily Living (LCADL) scale. six-minute walking test (6MWT). the Medical Research Council (MRC) scale and the BODE index was calculated. The total score and the percentage of the total score LCADL (LCADL %total ) were compared between patients of the four quartiles of the BODE using the Analysis of Variance test. The Spearman correlation coefficient was used to investigate the association between scores of LCADL and BODE index. Results: Patients had an average of FEV 1 %pred=37±12% and were on average 66±8 years-old. The LCADL %total correlated with the BODE index (r=0.65. p<0.05) as well as with the variables FEV 1 . dyspnea and walked distance in the 6MWT (r=-0.42. r=0.76 and r=-0.67. p<0.05. respectively). The comparison of the average scores of the LCADL %total between BODE quartiles 1. 2. 3 and 4. demonstrated that only the 4th quartile differed significantly from the others (p<0.05). Conclusions: ADL limitation has a strong association with the BODE index in patients with moderate to severe COPD and with three of the four variables that composes it. Resumo Contextualização: A Doença Pulmonar Obstrutiva Crônica (DPOC) é uma doença progressiva que reduz a capacidade funcional. comprometendo as atividades de vida diária (AVD). A morbidade e a mortalidade na DPOC apresentam uma estreita relação com a limitação funcional nesses pacientes. Objetivos: Verificar se há associação entre a limitação nas AVD e o índice preditor de mortalidade BODE em pacientes com DPOC moderada a muito grave e não moderada e grave. Métodos: Trinta e nove pacientes com DPOC GOLD 2 a 4. em uma amostra de conveniência. foram submetidos às avaliações: espirometria. índice de massa corpórea (IMC). escala London Chest Activity of Daily Living (LCADL). teste da caminhada de seis minutos (TC6min); escala Medical Research Council (MRC). e o índice BODE foi calculado. O escore total e o percentual do escore total da LCADL (LCADL %total ) foram comparados entre os pacientes dos quatro quartis do BODE utilizando-se ANOVA. O Coeficiente de Correlação de Spearman foi utilizado para verificar a associação entre os escores da LCADL e do índice BODE. Introdução: A doença pulmonar obstrutiva crônica (DPOC) é classificada atualmente como importante causa de morbidade e mortalidade no mundo e está associada a importantes manifestações sistêmicas. O body mass index airway obstruction, dyspnea and exercise capacity (BODE) é um índice prognóstico de mortalidade mais utilizado atualmente, pois avalia as manifestações respiratórias e sistêmicas da DPOC. Objetivo: Analisar, através do índice de BODE, os pacientes com DPOC severa participantes de um Programa de Reabilitação Pulmonar. Metodologia: Estudo documental, retrospectivo e quantitativo. Analisou-se o índice BODE e suas relações com outras variáveis fisiológicas. A amostra foi composta por 29 prontuários do Programa de Reabilitação Pulmonar, e investigaram-se os seguintes registros: idade, sexo, índice de massa corporal, VEF1, índice de dispneia modificada, Teste de caminhada de seis minutos, SF-36 e do índice BODE. Resultados: Dos pacientes analisados, 48,3% (n = 14) eram do sexo feminino e 51,7% (n = 15) masculino; idade 66 ± 8,8 anos; IMC 25,1 ± 4,4 kg/m². Verificaram-se as seguintes diferenças VEF1 0,98 ± 0,4 e 0,95 ± 0,3 (p = 0,26); MRC 2,40 ± 1,0 e 1,76 ± 0,6 (p = 0,005); BODE 4,0 ± 1,7 e 3,29 ± 1,1 (p = 0,009); TC 6min 404,58 ± 110,4 e 439,96 ± 97,3 (p = 0,02) antes e após o Programa de Reabilitação respectivamente. A correlação entre o índice BODE e o TC6’ foi r = -0,51 e do índice BODE e VEF1 foi r = -0,81. Conclusão: Pode-se inferir que o Programa de Reabilitação Pulmonar modificou, de forma significativa, o índice de BODE influenciando a qualidade de vida dos pacientes.

Abstract: Post-BD IC% predicted is the best functional predictor of distance walked and is significantly associated with GOLD staging and BODE index. Therefore, We propose that the inspiratory capacity should be added to the routine evaluation of the COPD patients.
