OBJECTIVES:Silicosis is a chronic and incurable occupational disease that can progress even after the cessation of exposure. Recent studies suggest that the forced oscillation technique may help to clarify the changes in lung mechanics resulting from silicosis as well as the detection of these changes. We investigated the effects of airway obstruction in silicosis on respiratory impedance and evaluated the diagnostic efficacy of the forced oscillation technique in these patients.METHODS:Spirometry was used to classify the airway obstruction, which resulted in four subject categories: controls (n = 21), patients with a normal exam (n = 12), patients with mild obstruction (n = 22), and patients with moderate-to-severe obstruction (n = 12). Resistive data were interpreted using the zero-intercept resistance (R0), the resistance at 4 Hz (Rrs4), and the mean resistance. We also analyzed the mean reactance (Xm) and the dynamic compliance. The total mechanical load was evaluated using the absolute value of the respiratory impedance (Z4Hz). The diagnostic potential was evaluated by investigating the area under the receiver operating characteristic curve. ClinicalTrials.gov: NCT01725971.RESULTS:We observed significant (p<0.0002) increases in R0, Rrs4, Rm, and Z4Hz and significant reductions in Crs,dyn (p<0.0002) and Xm (p<0.0001). R0, Rrs4, Rm, and Z4Hz performed adequately in the diagnosis of mild obstruction (area under the curve>0.80) and highly accurately in the detection of moderate-to-severe obstruction (area under the curve>0.90).CONCLUSIONS:The forced oscillation technique may contribute to the study of the pathophysiology of silicosis and may improve the treatment offered to these patients, thus representing an alternative and/or complementary tool for evaluating respiratory mechanics.

A tuberculose (TB) é uma das enfermidades mais antigas que existem na história. O primeiro relato documentado vem do século XIX, estando intimamente ligada às condições higiênico-sanitárias. O agente etiológico é o Mycobacterium tuberculosis e sua transmissão ocorre por via aérea. Atualmente, é uma patologia que enseja ainda atenção pela alta incidência entre as doenças infectocontagiosas e provoca impacto na saúde coletiva, tanto na forma tradicional (pulmonar), quanto na extrapulmonar. Os autores apresentam um caso de tuberculose pleural em paciente privado de liberdade, sob custódia judicial, diagnosticado no curso de suas internações para tratamento de fratura do fêmur e de osteomielite consequente a fratura exposta da tíbia. Além do relato, foi feita a análise deste caso baseado na literatura. A descrição do diagnóstico do paciente está intimamente atrelada a sua própria condição social. Fato este, que não deve ser exceção no contexto brasileiro. Inúmeras pessoas não aderem ao tratamento por falta de informação, perpetuando assim, a cadeia de transmissão. A metodologia do artigo trata-se de um relato de caso descritivo. Como principal resultado, pode-se afirmar que a investigação e conduta do paciente, foram alinhadas ao que se observa na literatura médica, consequentemente, chegou-se ao desfecho correto do problema. Algumas medidas simples, como a implementação de questionários para busca de SR e a busca radiológica para os casos suspeitos, levariam a uma potencial redução do número de pessoas infectadas por TB dentro dos presídios. Diversas peculiaridades podem estar atreladas a um diagnóstico, entre eles, a condição biopsicossocial do paciente. A propagação da informação ainda é um desafio muito grande a ser vencido, a educação em saúde faz-se necessária para que inúmeros indivíduos consigam entender a respeito de sua doença, sua cadeia de transmissão e sua resolução. Este artigo foi submetido e aprovado pelo comitê de ética em pesquisa da Universidade Federal do Amapá.

Análise das características hematológica do líquido ascítico de pacientes internos no Hospital Universitário Lauro Wanderley (HULW)

O objetivo desse trabalho foi analisar as características hematológicas dos líquidos ascíticos de pacientes internos no Hospital Universitário Lauro Wanderley assim como sua importância para determinar o diagnóstico. A população do estudo foi composta por pacientes com diversas patologias, admitidos no HULW, de ambos os gêneros, com idade igual ou superior a 18 anos com diagnóstico de ascite. O estudo foi realizado de acordo com as exigências do Comitê de Ética em Pesquisa (CAAE 23933819.0.0000.5183) do HULW. Foram analisados 309 exames. Destes, 68% eram do gênero masculino e a maior incidência na faixa etária acima de 60 anos (41%) com predomínio de ascite como diagnóstico estabelecido. A peritonite bacteriana esteve presente em 12% amostras e 6% apresentaram aspectos hemorrágico. Esses dados corroboram com a literatura, em que há o predomino do gênero masculino e a idade média 56,4 ± 6 anos, o que pode ser atribuída à prevalência de vírus da hepatite na população, bem como ao momento da disseminação da infecção viral, como também, o uso do álcool como fator relevante para desencadear a cirrose. Entre as limitações de nosso estudo, deve-se ressaltar a falta de informação clara no preenchimento da requisição do exame laboratorial, como também a coleta adequada e manuseio da amostra, o que dificulta e prejudica a análise dos fluidos cavitários.
