Background:Leprosy is a chronic granulomatous disease of the peripheral nerve and muscle of upper respiratory tract. Consequence to the neighboring primary site, oral lesions have been seen in 20--60% of patient with leprosy mainly lepromatous leprosy. The lepromatous nodules may be infective and may lead to the spread of disease and hence require proper diagnosis.Aim:To assess oral lesion in leprosy patients. To assess the disease and the occurrence of oral lesion according to age and gender. To detect any primary lesion in oral cavity and compare these lesions in duration to study.Materials and Methods:A total of 100 patients with leprosy were examined and there oral manifestations were recorded.Results:It was found that 70 (70%) leprosy patients showed oral manifestations. And 18 (25%) had chronic generalized periodontitis, 9 (12.8%) cases of oral melanosis, 7 (10%) atrophy of papillae and loss of taste sensation each, 6 (8.5%) complained of aphthous and candidiasis each, 5 (7.14%) depigmentation, and 4 (5.71%) smokers palate, oral submucous fibrosis, and fissured tongue.Conclusion:Our clinical findings are in conjugation with previous studies; however, as per the review of literature this is the first study worldwide where we have examined 100 cases of leprosy which has not been documented till date. We suggest that the lesser incidence of oral lesions observed recently compared to older reports because of more effective present treatment which is initiated earlier.

The inflammatory cytokines involved in the immune response to chronic periodontal disease (CPD) in the context of leprosy reactions (LR) were analyzed in 57 new cases of multibacillary leprosy (MBL). They were stratified by the presence of CPD and LR. Messenger RNA (mRNA) expression of inflammatory mediators was determined by qRT-PCR using skin biopsy and by ELISA using serum samples, maintaining 5% of significance level in ANOVA and correlation analyses. Twenty-three (40.4%) patients presented the first LR, whereas 22 (45.0%) patients presented CPD. IL-4 and IL-6 serum levels were significantly lower in patients with CPD and LR than in patients without CPD but with LR; IFN-γ serum levels were higher in patients with CPD and LR than in patients with no CPD and no LR; IL-4 serum levels were negatively correlated with TNF-α gene expression, while IL-6 serum levels were positively correlated with IFN-γ gene expression, in the skin of subjects with CPD and LR. The presence of DPC in individuals with LR immunoregulated IL-6, IFN-γ, and IL-4 concentrations. The presence of DPC decreased serum levels of IL-6 and IL-4 in reactional individuals. CPD concomitant to LR resulted in increased IFN-γ serum levels.

A hanseníase é uma doença infectocontagiosa que se manifesta principalmente através de sinais e sintomas dermatoneurológicos. Este estudo analisou o conhecimento e as práticas de profissionais de nível superior da equipe Saúde da Família (eSF) sobre a inter-relação entre a hanseníase e suas alterações bucais. Trata-se de um estudo transversal, observacional e analítico, com abordagem quantitativa. A amostra de conveniência compreendeu 24 médicos, 45 enfermeiros e 24 cirurgiões-dentistas (CDs) da eSF de um município do Nordeste do Brasil. O instrumento de coleta de dados foi um questionário estruturado com escalas do tipo Likert de 5 pontos. Baseado nos escores da pesquisa, interpretou-se que os profissionais possuem um conhecimento "insuficiente” sobre as alterações bucais na hanseníase e uma prática "adequada”. As evidências sinalizaram a necessidade do aprimoramento do conhecimento sobre a alterações bucais na hanseníase e organização das práticas profissionais de todos os envolvidos na eSF, pois para garantir o atendimento integral é fundamental despertar para a importância das práticas odontológicas no desenvolvimento das ações de controle a hanseníase na eSF.
